Objective: This study aims to explore the clinical effect of traditional Chinese medicine Tiaozhou method combined with lifestyle management in the treatment of polycystic Ovarian Syndrome-Insulin Resistance(PCOS-IR)compared with simple lifestyle management,To provide new ideas for the clinical treatment of patients with phlegm-dampness type PCOS-IR.Methods: From December 2018 to January 2020,71 cases of patients with phlegm-damp type POCS-IR were screened for gynecological outpatient screening at the First Affiliated Hospital of Tianjin University of Traditional Chinese Medicine from December2018 to January 2020.The patients were randomly divided into 36 cases in the treatment group and 35 cases in the control group.The treatment group was treated with oral Chinese medicine by the tutor Professor Zhu Ying’s self-adjusted weekly method,and lifestyle management was carried out,while the control group was purely lifestyle management.Both groups were treated continuously for 12 weeks,followed up for 8 weeks,while monitoring safety indicators and observing adverse reactions.58 cases were actually completed,30 in the treatment group and 28 in the control group.Main efficacy evaluation indicators: body mass index(BMI),waist-to-hip ratio(WHR),basic sex hormones(T,LH/FSH,PRL),insulin resistance indicators(FINS,FPG,HOMA-IR),four items of blood lipid(TC,TG),HDL-C,LDL-C)and TCM symptom scores,and calculate the basal body temperature biphasic rate and pregnancy rate during the treatment period and follow-up period of the patient.Use SPSS21.0 software for statistical analysis of data.result:(1)BMI and WHR: BMI and WHR decreased after treatment between the two groups,and the difference was statistically significant(P<0.05).The difference in BMI drop between the treatment group and the control group was higher than that of the control group,and the difference was statistically significant(P<0.05).(2)Basic sex hormones: the difference in T between the two groups after treatment in the two groups was statistically significant(P<0.05),and the difference in the treatment group LH/FSH was statistically significant(P<0.05).The difference between the LH/FSH drop in the treatment group was higher than that in the control group,and the difference was statistically significant(P<0.05).(3)Insulin resistance-related indicators: FINS and HOMA-IR in both groups decreased after treatment in the two groups,and FPG decreased in the treatment group.The difference was statistically significant(P<0.05).The difference between the two groups of patients before and after treatment was compared between the two groups,and the difference was not statistically significant(P>0.05).(4)Four items of blood lipids: the comparison of TC decreased after treatment between the two groups,and the difference in LDL-C decrease in the treatment group was statistically significant(P<0.05).The difference between the two groups of patients before and after treatment was compared between the two groups,and the difference was not statistically significant(P>0.05).(5)TCM symptom score: After comparing the two groups of patients,the TCM symptom score decreased after treatment,and the difference was statistically significant(P<0.05).Comparison between groups The difference in the decrease in TCM symptom scores of the treatment group was higher than that of the control group,and the difference was statistically significant(P<0.05).(6)Biphasic rate of basal body temperature: The biphasic rate of BBT increased during the treatment period in both groups(58.11% vs 40.91%),and the difference was statistically significant(P<0.05).The biphasic rate of BBT during the follow-up period of the two groups(73.91%vs60.47)The difference was not statistically significant(P>0.05).(7)Pregnancy rate: There was no statistically significant difference in the pregnancy rate of the two groups of patients during the treatment period(13.64% vs 5.26%)(P>0.05),and there was no significant difference in the pregnancy rate of the follow-up period(10.53% vs5.56%)(P>0.05).Conclusion: Lifestyle management can reduce BMI,WHR and insulin-related indicators of patients with phlegm-dampness PCOS-IR,effectively reduce T,improve blood lipid levels,reduce TC,and improve clinical symptoms.Compared with simple lifestyle management,lifestyle management combined with Chinese medicine weekly adjustment method can better reduce patients’ BMI,lower LH/FSH ratio of basic sex hormones,lower FPG,and also lower LDL-C,which can improve patients’ basal body temperature.Biphasic rate,improve patients’ clinical symptoms such as menstrual cycle and menstrual flow.
