Objective: To preliminary explore the distribution rules of TCM syndromes in attention deficit hyperactivity disorder with epilepsy,summarize its core pathogenesis,and effectively guide clinical syndrome differentiation treatment.Methods: This study used a retrospective study design method to include 100 children with epilepsy comorbid attention deficit hyperactivity disorder who were treated in the Pediatric Encephalopathy Outpatient Clinic of the First Affiliated Hospital of Tianjin University of Traditional Chinese Medicine,according to Zhu Wenfeng's Syndrome Differentiation In the "Syndrome Differentiation Scale",a clinical questionnaire was developed to collect information on the child's general condition,clinical manifestations,auxiliary examinations,treatment and related syndromes,and was entered into an Excel table.SPSS 20.0 software was used for statistics and analysis of epilepsy.Distribution of TCM syndromes in children with attention deficit hyperactivity disorder and its influencing factors.Result:(1)General situation:(1)Sex and age: This thesis included a high proportion of 100 boys with epilepsy comorbid attention deficit hyperactivity disorder.There were 29 cases(29%)in children aged 4 to 6 years,60 cases(60%) in children aged 7 to 12 years,and 11 cases(11%) in children aged 13 to 14 years,mainly among school-age children.(2)Mode of production and perinatal period: The number of children with cesarean section was 1.7 times that of normal delivery.There were 52 cases with abnormal perinatal period,including 23 cases with umbilical around the neck.(3)Family factors: 19 cases of abnormal family history,including 8 cases of febrile seizures,accounting for 42%;parents with undergraduate and undergraduate accounting for 63%;67 cases of comorbid children with only children,accounting for 67%.(4)Concomitant epilepsy: 41 cases(41%) of comorbid children with epilepsy age ? 3 years old,28 cases(28%) 4-6 years old,28 cases(28%) 7-12 years old,3 cases> 12 years old(3%),the age of first onset was concentrated in early childhood;the shortest course of epilepsy in comorbid children was 6 months and the longest 11 years,of which 33 cases(33%) were less than 1 year,the proportion was high;the type of seizures was rigid-flame There were 48 cases of contracture seizures,20 cases of absence seizures,17 cases of simple partial seizures,12 cases of autonomic seizures,and 3 cases of atonic seizures.The proportion of tonic-clonic seizures was high,accounting for 48%.(5)In terms of EEG: 19 cases of normal EEG,81 cases of abnormality,67 cases with specific description of abnormality,of which 4 cases(4%) mainly showed sharp waves and spike waves,and 5 cases mainly showed slow waves(5%),58 cases(58%) were dominated by sharp slow waves,spike slow waves,and multiple spike slow waves,of which mixed waves accounted for the largest proportion.(6)Concomitant ADHD: 24 cases of attention deficit type,15 cases of hyperactivity-impulsive type,and 61 cases of mixed type.Among them,mixed type accounts for the highest proportion,accounting for 61%;attention deficit type is significantly related to gender and age of first epilepsy;mixed type Related to the cause of fever.(7)Medication: 39 cases only took Chinese medicine decoction,accounting for 39%;32 cases of Chinese medicine combined with 1 kind of anti-epileptic western medicine,2 kinds of 20 cases,and 3 kinds or more of 9 cases.(2)Analysis of syndromes:(1)14 kinds of pathogenic syndromes meet the requirements,and the results of frequency statistical analysis are from high to low: closed(81 cases)>moving wind(78 cases)> phlegm(54 cases)> Qi deficiency(40 cases)> Blood deficiency(30cases)> Yin deficiency(26 cases)> Infirmity(12 cases)> Wet(7 cases)> Hyperactivity(6 cases)> Yang deficiency(6 cases)> Heat(2 cases)> Qi Stagnation(1 case)> External wind(1case),mainly concentrated on empirical factors such as closure,moving wind,phlegm and other factors such as Qi deficiency,blood deficiency and Yin deficiency.(2)There are 8 kinds of pathological syndromes that meet the requirements,and the frequency statistical analysis results are from high to low: Xinshen(99 cases)>Liver(79 cases)>Kidney(11 cases)>Lung(10 cases)>Table(8 cases)> Meridian(7 cases)> Spleen(5 cases)> Stomach(5 cases),mainly concentrated in the mind,liver and kidney.(3)Less than 3 kinds of syndromes account for 25%,4-6 kinds of syndromes account for 54%,7-10 kinds of syndromes account for 26%,and more than 10 kinds of syndromes account for 5%.(4)According to the analysis of pathogenic syndromes,the evidence is 47%,the syndrome is 2%,and the syndrome is 51%.(3)Analysis of factors influencing syndrome:(1)Phlegm syndrome is related to headache.(2)The closure of the syndrome is related to the cause of seizure fever and the age of onset of epilepsy.(3)Qi deficiency syndrome is related to the history of febrile seizures,susceptibility to normal sleep,and poor sleep quality.(4)Blood deficiency syndrome is relatedto the number of antiepileptic western medicines,poor sleep quality,and abnormal stool.(5)Yin deficiency syndrome is related to poor sleep quality and susceptibility.(6)The liver syndrome is related to vomiting.Conclusion:(1)In children with epilepsy comorbid attention deficit hyperactivity disorder,disease-related syndromes are mainly concentrated in closure,wind,phlegm,qi deficiency,blood deficiency,and yin deficiency.The syndrome of deficiency and excess is more common,and the disease-location syndromes are mainly concentrated in mind(brain),Liver and kidney.(2)Through syndrome analysis,the pathogenesis of children with attention deficit hyperactivity disorder with epilepsy is mainly liver and kidney yin essence deficiency,resulting in hyperactivity of liver yang,liver phlegm disturbance,obstruction of mind,clinical treatment and nourishing Liver and kidney,phlegm and wind mainly.
