Alcohol dependence syndrome refers to long-term drinking,uncontrolled drinking,and a strong desire for alcohol,which can lead to a series of pathological behaviors of the body,which is a chronic recurrent disease,has risen to a society at present.Issues of concern from all walks of life.Alcohol dependent patients are often associated with other mental disorders,especially depression and anxiety see a lot of illness.Alcohol dependence and depression and anxiety often increase the risk of repeated drinking.Topiramate as a new kind of type antiepileptic drugs have been widely used in clinic since they were put on the market.In addition to antiepileptic effect,there are many therapeutic effects in clinic,among which alcohol craving can be reduced in alcohol dependent patients,but it can improve anxiety and depression.In terms of mood,the curative effect is not good,and these negative emotions are the main factors leading to repeated drinking.Escitalopram belongs to SSRIs antidepressants can improve the anxiety,irritability and depression of alcohol dependent patients after stopping drinking,and can relieve the nervousness and inattention in the side effects of topiramate.But the two are combined.The study of drug use has not yet been reported.Therefore,topiramate and escitalopram were used in this study.To observe the clinical effect of alcohol dependence patients with anxiety and depression symptoms in terms of sex,cognitive function and drinking rate.Curative effect.To further find a more effective treatment to reduce the psychological dependence of alcohol on alcohol dependent patients,so as to find a more effective way to reduce the psychological dependence of alcohol dependent patients.Reduce repeated drinking to provide basis and support.Objective:Clinical treatment of alcohol dependent patients with anxiety and depression treated with escitalopram combined with topiramate it works.Methods:The subjects selected alcohol hospitalized in the sixth people's Hospital of Hebei Province from January 2018 to December 2019.Dependent patients,all accompanied by anxiety and depression symptoms.128 patients in the control group were enrolled in a randomized,controlled clinical trial.The patients were divided into two groups:escitalopram combined with topiramate group(combined treatment group)and topiramate alone group(single drug group).A total of 117 patients withdrew from the treatment in the middle of the treatment and finally completed the experiment.There were 55 cases in the single drug group and 62 cases in the combined treatment group.The patients were treated for 8 weeks and followed up to 6 months.Before treatment patients with CIWA-Ar,were assessed with a score of less than 7 points and began to be treated after the score was less than 7 points.Before and after treatment,Hamilton depression scale(HAMD),Hamilton anxiety scale(HAMA),side effect scale(TESS),repeated neuropsychological status test(RBANS)and visual thirst scale(VAS)were used to evaluate the clinical efficacy and safety of the patients.Results:1.At the end of 4th and 8th week after treatment,the scores of Hamilton anxiety scale(HAMA)and Hamilton depression scale(HAMD)in the combined treatment group were significantly lower than those in the baseline score,the difference was statistically significant(p<0.05),the scores of HAMA and HAMD in the single treatment group were not significantly lower than those in the baseline at the end of the 4th and 8th week after treatment.The reduction rate of HAMA and HAMD was more than 50%in the combined treatment group,and the HAMA and HAMD score in the single drug group were less than50%.2.With the prolongation of treatment,the VAS score of both groups decreased gradually,and the craving degree of the combined treatment group decreased even more obvious.The drinking rate in the combined drug group was lower than that in the single drug group at 6 months follow-up,and the difference was statistically significant(?~2=13.008,p=0.000<0.05).The adverse reactions in the combined drug group were better than those in the single drug group the incidence of should be low.Conclusion:1.The efficacy of escitalopram combined with topiramate in the treatment of alcohol-dependent patients with anxiety and depression symptoms was better than that of topiramate alone.2.Escitalopram combined with topiramate can reduce the patients'thirst for alcohol,the rate of rehydration and the incidence of adverse reactions.
