Background: Alcohol dependence is a state of psychological hobby and physiogenic drug addiction after a long time intake of larger alcohol. Alcoholics have to drink alcohol regularly because of either obtain satisfied hobby or refrain from body unwell reaction for stop drinking. Along with the increase amount of alcoholics, there are more alcoholics were to be hospitalization for treatment, but after treatment in hospital, many of them relapse when post-discharge and than dependence again. The rates of relapse can reach up to 83.6% in some studies. There are many researches on alcohol dependence and studies with regard to relapse reason and correlation factors. It is doctors and a lot of alcoholics family members are concerned about what kind of alcoholics would be relapse .Some study discovered that many factors could promote alcoholics relapse that include of low cultural level and physical labor person, more life events, comparatively short of length time of stay in hospital, personality change due to alcohol use. or people with positive family history of alcohol dependence, social psychological stress, late onset age of alcohol dependence, long time of drunkenness, or patients of alcoholic delusiveness. While these researches are predicted the relapse factors of alcoholics from the social psychological factor, the study of relationship about cognitive function and relapse of alcoholics is scarce. And we explore the relapse factors from the point of view of Biology.Objective: To investigated the cognitive function of alcoholics of first hospitalization inpatient and inpatient readmission, to study their brain C, electroencephalogram, degree of alcohol dependence and comorbidity of other mental disease, to explore the predicted factors of drinking relapse of the alcoholics.Method: 32 inpatients of alcohol dependence and 28 normal control persons were measured. 32 inpatients of alcohol dependence in Department of Psychiatry of First Hospital of Hebei Medical University were investigated. For all the patients, we examine their skull CT, electroencephalogram, Michigan Alcoholism Screening Test (MAST) and a general condition research card when they were to be in hospital, after four weeks treatment, the patient was measured with some type of cognition scales, for example: Wisconsin Card Sorting Test, Tower of London test,Stroop Color–Word task and other tests, record the dates. The alcohol dependence group was divided into two groups based on their hospitalization time: first hospitalization group (FG) and readmission group (RG). To analysis cognitive function of two alcoholics groups, To compare their brain CT, electroencephalogram (EEG), comorbidity of other mental disease, the degree of alcohol dependence between the first hospitalization patients and those of readmission patients. And age, culture level, the age of begin drinking, the age of onset of dependence, the length of dependence time and average intake alcohol volume per day were also analysis between two alcoholic groups. Control group is selected form patient's accompany or staff of our hospital matched with alcoholics group in age and cultural level. There was no history of psychosis and other substance abuse (except tobacco use) in control group. And control group was measured just with the same cognition scales as those alcoholics group were used.Result: 1 The results of neuropsychological tasks of alcohol dependence patients were significant lower than those of the control group. The scales of all cognition tests except Digit span test and Space span test was significant lower than that of the control group.2 The scales of neuropsychological tasks of first hospitalization group were significant higher than those of readmission group. Those test including of Verbal memory test, Continuous procedure test, Stroop color word test, Space span test, Digit sequence test and Tower of London test; There is no significance between two alcoholic groups in the scales of Visual memory test and Digit span test. And the scores of WSCT is no difference in both alcoholic groups.3 Abnormal rate of skull CT in readmission group was significant higher than that in the first hospitalization group.4 Abnormal rate of electroencephalogram in readmission group was significant higher than that in first hospitalization group.5 The rate of comorbidity with other mental disease in both alcoholic groups was no difference.6 There were no different between two alcohol dependence groups on dependence degree of scales of MAST.7 Readmission group have older age, more drinking volume daily, longer time of dependence, late onset of disease (dependence), low cultural standard. But there is no significant between two groups.Conclusion: 1 Cognitive function of patients of abstain alcohol dependence is serious impairment especially those repeatedly hospitalization, but there are a part of the cognitive function remained.2 Brain formation and brain electrophysiology is damage for most alcohol dependence patients, the readmission patients is worse than those of first hospitalization.3 There were no significant effect on relapse with dependence degree of alcohol and comorbidity with other mental disease.4 It is very important to test the alcoholic's cognitive function and check out electroencephalogram and brain CT, if the cognitive function is damage and the brain CT and electroencephalogram is abnormal, then we can conclude that this patients is easy to relapse after abstain, and we should do something to prevent this happen as soon as possible. |
