Study On The Relationship Between Adult Attachment,Coping Style And Mental Health In Breast Cancer Patients

ObjectiveTo understand the status of adult attachment in breast cancer patients,to explore the relationship between different attachment types,disease coping style and mental health level of breast cancer patients,to find the regulatory factors of mental health,improve the level of mental health of patients.MethodsUsing the convenience sampling method,273 patients with breast cancer from January 2018 to March 2019 were selected from grade III first-class hospital in Anhui province.The Simple Coping Style Questionnaire(SCSQ),Intimate relationship Experience Scale(ECR)and psychological stress Rating Scale(K10)were used to investigate.Variance analysis and chi-square test were used to compare the differences of each variable in general demographic characteristics.Pearson analysis was used to investigate the correlation between adult attachment,coping style and mental health.To explore the effect of adult attachment style on mental health of breast cancer patients by disordered multiple logistic regression model.Hayes’ PROCESS program was used to explore the mediating effect of coping style on adult attachment style and mental health.Results1.The average score of adult attachment intimacy experience scale for breast cancer patients was(143.50±23.11),among them,attachment avoidance(63.44±11.34)and attachment anxiety(75.86±16.79).After the conversion of the dimension scores,27cases(9.89%),174 cases(63.71%),45 cases(19.41%)and 27 cases(9.89%)were found to be secure,fearful,attentive and indifferent.The average score of psychological stress rating scale was(26.93±6.90).The questionnaire on simple coping style was(31.68±7.78),including positive coping style was 19.80±5.79 and negative coping style was 11.88±3.91.2.Pearson correlation analysis showed that attachment anxiety and attachment avoidance were positively correlated with mental health(r=0.310,0.328,p<0.01).Positive coping was negatively correlated with mental health(r=-0.386,p<0.01),negative coping was significantly positively correlated with mental health(r=0.232,p<0.01).Attachment anxiety was negatively correlated with positive coping(r=-0.136,p<0.05)was positively correlated with negative response(r=0.295,p<0.01).Attachment avoidance was negatively correlated with positive response(r=-0.156,p<0.01),positively correlated with negative response(r=0.229,p<0.01).Mental health was positively correlated with fear-based attachment style and negative coping style(r =0.392,0.232,both p<0.01),and negatively correlated with positive coping style(r=-0.386,p<0.01).Fear-based attachment style was negatively correlated with positive coping style(r =-0.177,p< 0.01)and positively correlated with negative coping style(r =0.334,p< 0.01).2.With multiple classification logistic regression,taking secure type as reference,results showed that the OR value of mental health effect on fearful patients,attentive patients and indifferent patients was 1.235,1.227 and 1.222 respectively.4.The mediating test showed that positive coping had a mediating effect between attachment avoidance and mental health,and the mediating effect accounted for 16.3%of the total effect.The ratio of the mediating effect of negative coping between attachment anxiety and mental health was 16.6%.Positive coping played a partial mediating role between fearful attachment style and mental health,and the ratio ofmediating effect to total effect was 16.3%,while negative coping played a complete mediating role between fearful attachment style and mental health.ConclusionThe adult attachment status of breast cancer patients is poor,among whose the mental health status of fear type,focus type and apathy type is poor,and the mental health status of fear type attachment style patients is adjusted by coping style.Therefore,at the beginning of breast cancer patients in hospital should be based on attachment style,behavior and communication features simple judging patients on the basis of attachment style such as peer support education,spouses synchronous health education,parent-child activities such as group intervention and individualized counseling,guide the different characteristics of patients with cognitive,change the coping styles,breast cancer patients psychological returned to normal.
