Clinical Observation Of Nasal Cavity Flushing With Modified Buyang Huanwu Decoction Afer FESS With Chronic Rhinosinusitis

Objective The target is to investigate the clinical effect of Modified Buyang Huanwu Decoction on the recovery of nasal mucosal function after FESS in patients with chronic rhinosinusitis.Methods From September 2016 to December 2017,120 patients who met the inclusion criteria were randomly divided into three medication groups.The first one is Triamcinolone Acetonide Nasal Spray Group(hereinafter referred to as Triamcinolone Acetonide Group).Chinese medicine nasal irrigation and Triamcinolone Acetonide group(hereinafter referred to as Chinese medicine group)are another component.The last part consists of Normal Saline Rinse and Triamcinolone Acetonide Group(referred to as Saline Group).All patients after FESS routinely took nasal dressing change.And triamcinolone acetonide nasal spray was used in the triamcinolone acetonide group.Meanwhile,the nasal cavity was sprayed on the side of the operation every morning and evening.In the traditional Chinese medicine group,Buyang Huanwu Decoction was added or subtracted for nasal irrigation.It was decocted twice every 2 days.The decoction was filtered and decocted for 400 ml.The nasal irrigation was done four times every 2 days.After irrigation,triamcinolone acetonide nasal spray was used to spray the nasal cavity on the side of operation every morning and evening.In the saline group,400 ml of saline was used to rinse the nasal cavity four times in two days,100 ml each time.Then triamcinolone acetonide nasal spray was used to spray the nasal cavity on the side of operation every morning and evening.The three groups were treated for 8 weeks.The scores of nasal obstruction,pus,headache,hypoolusia and other symptoms were recorded in 120 patients before,on the 8th,15 th,36th and 56 th days of treatment.The scores of nasal endoscopic signs and the time of saccharin clearance were recorded before and 56 days after treatment.To evaluate the efficacy of the two methods,and then we analyzed the efficacy of triamcinolone acetonide group,traditional Chinese medicine group and saline group.Results 1.After 8 weeks of treatment,the severity of symptoms and signs was scored and the curative effect was evaluated.The results were as follows: The total effective rate of triamcinolone acetonide group was 77.5%.The total effective rate of Chinese medicine group was 92.5%.The total effective rate of saline group was 85%.The effective rates of the three groups were different(P < 0.05).It can be seen that the total effective rate of traditional Chinese medicine group is higher than that of saline group and triamcinolone acetonide group.2.In saccharin clearance time: Through statistical analysis,it can be seen that the three groups are effective after treatment.The comparison results of the three groups show that the reduction of saccharin clearance time in traditional Chinese medicine group is better than that in triamcinolone acetonide group and saline group.What's more,the saline group was better than triamcinolone acetonide group.3.The VAS scores of the three groups were compared.After 8 weeks,the total VAS scores were significantly reduced.There were statistical differences in nasal obstruction and runny nose among the three groups.And the improvement effect of each index in the traditional Chinese medicine group was higher than that in the triamcinolone acetonide group and the saline group.4.The results of nasal endoscopy scores of the three groups were compared.After 8 weeks,the total score of nasal endoscopy was significantly reduced.There was no difference in the index of nasal polyps(no recurrence was found in the three groups of patients with preoperative polyps after treatment).There were statistical differences in the efficacy of edema and scab formation between the three groups(P < 0.05).The improvement effect of each index in the traditional Chinese medicine group was higher than that in the triamcinolone acetonide group and the saline group.Conclusion 1.All three methods,to some extent,it has curative effect on reducing TCM symptom integral and nasal endoscopy sign integral.2.The improvement effect of traditional Chinese medicine group is better than that of triamcinolone acetonide group and saline group,which is worthy of clinical application.
