ObjectiveThrough relatively reasonable clinical research,the general information of patients with lung cancer fever,symptoms of four diagnostic methods of traditional Chinese medicine and TCM syndrome distribution were analyzed,and the related influencing factors of TCM Syndromes of patients with lung cancer fever were further analyzed,and the distribution characteristics of TCM Syndromes of patients with lung cancer fever were roughly summarized,so as to provide reference for clinical identification,TCM syndrome judgment and prognosis of patients with lung cancer fever TCM syndrome differentiation provides objective basis.MethodsFrom January 2018 to January 2020,patients with lung cancer and fever in the respiratory and oncology wards of the Second Affiliated Hospital of Tianjin University of traditional Chinese medicine were collected.According to the information questionnaire of patients with lung cancer and fever,we collected the general situation of patients,symptoms and signs of traditional Chinese medicine and other clinical information.We used Excel 2013 to input and summarize the data.Spss26.0 statistical software was used to analyze and process the data using descriptive statistics,cluster analysis,chi square test and other statistical methods.Results1 General data analysisIn this study,85 cases of lung cancer fever patients were collected,mainly elderly male patients,64 cases(75.3%)were male patients,21 cases(24.7%)were female patients;the age distribution of patients was 30-88 years old,9 cases(10.6%)were under 60 years old,and 76cases(89.4%)were?60 years old.There were 54 cases(63.5%)with low fever,22 cases(25.9%)with moderate fever and 9 cases(10.6%)with high fever.There were 47 cases(55.3%)with definite pathological type,including 23 cases of lung adenocarcinoma and 19 cases of lung squamous cell carcinoma.38 cases(44.7%)were clinically diagnosed.The clinical stage of the patients was mainly in the middle and late stage,with 2 cases(2.4%)instage I,10 cases(11.8%)in stage II,38 cases(44.7%)in stage III and 35 cases(41.2%)in stage IV.There were 35 cases(41.2%)of hypertension and 38 cases(44.7%)of coronary heart disease.In the personal history survey,66 cases(77.6%)had smoking history,among which54 cases(63.5%)were male smokers;31 cases(36.5%)had drinking history;12 cases(14.1%)had drug allergy history;14 cases(16.5%)had environmental contact history;15 cases(17.6%)had family history of cancer.2 Study on the distribution of TCM SyndromesAccording to the statistics of the frequency of the four symptoms in TCM,the order of the frequency of occurrence is cough,asthenia,thick phlegm,slippery pulse,little phlegm,white phlegm,wheezing,dry stool,white moss,purple dark tongue or bruised spots or cyanosis,pulse string,fine pulse,and fatigue.The main TCM syndromes were Qi deficiency syndrome(67 cases),yin deficiency syndrome(44 cases)and blood stasis syndrome(40cases),which accounted for 78.8%,51.8% and 47.1% of the total number of patients,followed by blood deficiency syndrome(29 cases),phlegm dampness syndrome(26 cases)and phlegm heat syndrome(19 cases),which accounted for 34.1%,30.6% and 22.4% of the total number of patients,and finally Yang deficiency syndrome(4 cases)and qi stagnation syndrome(5 cases),which accounted for 47% and 5.9%.According to the statistics of syndrome differentiation frequency of Zang Fu organs,there were 15 cases in the heart(17.9%),80 cases in the lung(95.2%),30 cases in the spleen(35.7%),18 cases in the stomach(21.4%),16 cases in the liver(19.0%),15 cases in the kidney(17.9%).The R-type systematic cluster analysis was carried out for 69 TCM symptoms with a frequency of more than 10%,and five syndrome types were obtained,namely Yin deficiency and fluid deficit,phlegm-stasis syndrome,Yin deficiency and blood deficiency,qi and blood deficiency,and lung and spleen qi deficiency.3 Study on the correlation of TCM SyndromesThere was no significant difference between gender and TCM Syndrome Distribution in85 cases of lung cancer fever patients in this study(P>0.05).The distribution frequency of TCM syndromes in each group was statistically analyzed,showing that there was a statistical correlation between blood deficiency syndrome and gender(P<0.05).There was no significant statistical correlation between the distribution of TCM syndromes and age inpatients with lung cancer fever(P>0.05).There was no significant correlation between the distribution of TCM syndromes and the degree of fever in lung cancer patients with fever(P>0.05).There was no significant statistical correlation between TCM Syndrome Distribution and pathological types in patients with lung cancer fever(P>0.05).There was no significant statistical correlation between the distribution of TCM syndromes and clinical stages(P>0.05).There was no significant correlation between TCM Syndrome Distribution and smoking history in patients with lung cancer fever(P>0.05).Conclusion1 The majority of PLC patients with NF were elderly men,the majority of them were middle and advanced lung squamous cell carcinoma and lung adenocarcinoma.2 The TCM syndromes of patients with PLC and NF fever are mainly qi deficiency syndrome,Yin deficiency syndrome and blood stasis syndrome.The disease is mainly located in the lung,which is related to the spleen,kidney,stomach,heart and liver organs.3 Through cluster analysis,69 high-frequency symptoms can be clustered into five TCM syndrome types: Yin deficiency and Jin deficiency syndrome,phlegm and blood stasis syndrome,yin and blood deficiency syndrome,Qi and blood deficiency syndrome,lung and spleen qi deficiency syndrome.4 85 cases of lung cancer patients with fever in TCM syndrome,blood deficiency syndrome and gender were statistically correlated,blood stasis syndrome and fever degree was statistically correlated.
