Chief Physician Gu Shisong's Prescription Rule And Experiences In The Treatment Of Cardiac Neurosis

Objective: To study the outpatient medical records of Gu Shisong,the chief physician,in the treatment of cardiac neurosis,to explore the law of drug use,to analyze the thinking of syndrome differentiation and treatment,to summarize the academic thought,and to apply it to clinical practice to improve the curative effect,so as to provide a new thinking reference for the treatment of cardiac neurosis by traditional Chinese medicine.Methods: according to the inclusion and exclusion criteria,218 effective prescriptions of traditional Chinese medicine were collected from 75 patients who met the diagnosis of cardiac neurosis in the outpatient department of Gu Shisong,chief physician of psychosomatic department,Affiliated Hospital of Tianjin Institute of traditional Chinese medicine from December 2017 to October 2019.After the data standardization,the data is input into the database by EXCEL form,and the gender composition of patients and the frequency,four natures,five tastes,meridians and efficacy of each Chinese medicine in the outpatient prescription are counted,and the results are made into a chart.The high-frequency drugs in the prescription were introduced into spss24.0 software for cluster analysis,and a new combination of traditional Chinese medicine was obtained.This paper analyzes the frequency of the use of the nature,taste,meridian tropism and efficacy of each traditional Chinese medicine and the composition and drug pairs obtained by cluster analysis,and summarizes the medication rules and experience ideas of chief physician Gu Shisong in the treatment of cardiac neurosis for clinical application.Result:1.In the patients treated by Mr.Gu Shisong,the percentage of male was 25.33%,and that of female was 74.67%.The percentage of female was significantly higher than that of male.2.In the effective prescriptions for the treatment of cardiac neurosis,88 traditional Chinese medicines were used,3467 times in total,and 17 Chinese herbs were used more than 60 times,including Ramulus Cinnamomi,Radix Glycyrrhizae,Rhizoma Zingiberis,Ossa Draconis,Concha Ostreae,Radix Aconiti Lateralis Praeparata,Poria with hostwood,Semen Ziziphi Spinosae,Caulis Polygoni Multiflori,Magnetitum,Fructus Amomi,Concha Margaritifera Usta,Cortex Cinnamomi,Fructus Psoraleae,Rhizoma Cyperi,Fructus Evodiae,Dens Draconis.3.The use frequency of the four Qi from high to low is: flat,warm,cold,hot and cool.4.The order of the frequency of five flavors is: sweet,pungent,bitter,salty,sour,astringent and light.5.The frequency of meridian tropism from high to low is: heart channel,liver channel,spleen channel,kidney channel,lung channel,stomach channel,gallbladder channel,bladder channel,large intestine channel,triple energizer channel,Pericardium Channel.6.The first five kinds of efficacy frequency from high to low are: tranquilizing drug,warming medicine,tonifying medicine,resolving exterior medicine,calming liver and calming wind medicine.7.The data were imported into spss24.0 software for cluster analysis,and 7 groups of drug combinations were obtained: group 1: Radix Aconiti Lateralis Praeparata,Rhizoma Zingiberis,Radix Glycyrrhizae;group 2: Ramulus Cinnamomi,Ossa Draconis,Concha Ostreae;group 3:Poria with hostwood,Semen Ziziphi Spinosae;group 4: Caulis Polygoni Multiflori,Magnetitum;group 5: Dens Draconis,Concha Margaritifera Usta;group 6: Cortex Cinnamomi,Fructus Psoraleae;group 7: Fructus Evodiae,Rhizoma Cyperi,Fructus Amomi.Conclusion:1.According to Gu Shisong,the chief physician,the occurrence of cardiac neurosis is mostly related to factors such as improper medicine and food,emotional disorder,physical deficiency and fatigue;the basic pathogenesis is deficiency of heart Yang and uneasiness of mind;the disease location is mainly in the heart,which is related to the three viscera of liver,spleen and kidney;the disease nature is mainly deficiency syndrome,while the deficiency is mostly Yang deficiency;the basic pathogenesis is deficiency of heart Yang,floating of mind and spirit,and the treatment method is mainly warm latent method,warm latent method Invigorating the heart Yang and calming the nerves.2.Gu Shisong's main prescription for the treatment of cardiac neurosis is Guizhi licorice keel oyster soup and Sini decoction.When palpitation is accompanied by insomnia symptoms,add Poria with hostwood,Semen Ziziphi Spinosae,Caulis Polygoni Multiflori,Magnetitum;when there are symptoms of anxiety,tension,compulsion,fear and even shaking hands,add Dens Draconis,Concha Margaritifera Usta;when the waist and knees are sore,cold limbs are cold, urination is clear and long,tongue is pale,moss is white and pulse is heavy,etc When the symptoms of kidney yang deficiency are obvious,Cortex Cinnamomi and Fructus Psoraleae are added;when the daily phobia is accompanied by the depression of liver Qi caused by high mental pressure,Fructus Evodiae,Rhizoma Cyperi and Fructus Amomi can be added.3.Gu Shisong used heart body therapy in the treatment of cardiac neurosis,combined with Morita therapy and Chinese herbal decoction for psychological care.
