Risk Factors For Children With Functional Constipation And Major Types Of Traditional Chinese Medicine Syndromes

Objective: A retrospective analysis was conducted on the data of children with functional constipation(FC)and children without FC to explore the risk factors of FC and its main syndrome types in children,so as to provide references for the prevention and treatment of FC in children.Methods: Children admitted to the pediatric outpatient department of the first teaching hospital of Tianjin University of Traditional Chinese Medicine from March 2019 to November 2019 were selected as the study subjects,and were divided into case group and control group according to whether they were diagnosed with FC.Adopt the method of questionnaire survey,the application design of children functional constipation disease risk factors questionnaire,children's functional constipation in the questionnaire of syndromes,record the patient's general characteristics(gender,age,nationality,height,weight,etc.),bowel habit(stool-holding behavior,defecation time,concentration during defecation,the establishment of defecation regularity),feeding methods,eating habits,water intake,psychological factors,family history,personal history,physical activity,and the clinical symptoms and signs of TCM.The risk factors of FC in children and the risk factors of the main syndrome types in the case group were analyzed by means of case-control study.Results: By November 2019,208 children were enrolled,106 in the case group and 102 in the control group.(1)The single factor analysis showed that children's FC was no significantly associated with sex,age,nationality,height,weight,concentration during defecation,feeding methods,eating habits,insufficient water intake,psychological factors,personal history and physical activity(P > 0.05),and was significantly associated with whether the children with stool-holding behavior,prolonged defecation,the establishment of defecation regularity,family history of constipation(P < 0.05).(2)Multivariate logistic regression analysis showed that children with stool-holding behavior(OR=17.528,95%CI=7.114?43.169,P=0.000),first-level family history of constipation(OR=2.325,95%CI=1.112?4.864,P=0.025)are independent risk factors for FC in children,and the establishment of defecation regularity(OR=0.276,95%CI=0.131?0.581,P=0.001)is protective factor for FC in children.(3)Among the 106 children with FC in the case group,68(64.15%)children met the criteria of dyspepsia constipation,and 38(35.85%)children met the criteria of dryness-heat constipation.No children met the criteria of qi-stagnation constipation,qi-deficiency constipation and blood-deficiency constipation.(4)Children with stool-holding behavior is a risk factor for dyspepsia constipation(OR=17.524,95%CI=6.793?45.211,P=0.000)and dryness-heat constipation(OR=15.693,95%C I=5.185?47.498,P=0.000),and the the establishment of defecation regularity is a protective factor for dyspepsia constipation(OR=0.380,95%CI=0.170?0.847,P=0.018),and dryness-heat constipation(OR=0.166,95%CI = 0.048?0.575,P=0.005).Conclusion:(1)Children with stool-holding behavior,and the first-level family history of constipation are independent risk factors for FC in children,the establishment of defecation regularity is a protective factor for FC in children.(2)Children's FC syndrome types are mainly dyspepsia constipation and dryness-heat constipation,children with stool-holding behavior is the risk factor of the main syndromes in childhood FC,and the establishment of defecation regularity is the protection factor of the main syndromes in childhood FC.(3)The risk factors of the main syndrome types are similar to the risk factors of the FC,and the specificity of the risk factors of the various syndromes is not found.
