Research On The Construction Of Whole Process Management Model For Patients With Inflammatory Bowel Disease Based On Cloud Platform

[Objective] Use the Internet information technology to build a whole process management model for patients with inflammatory bowel disease based on cloud platform,design and develop a whole process management cloud platform to achieve efficient,convenient,and intelligent management for patients with inflammatory bowel disease.Provide references for exploring "Internet plus chronic disease management" and the whole process management for patients with inflammatory bowel disease.[Methods] This study taken the chronic care model and technology acceptance model as the theoretical basis,and provided a basis for the semi-structured interview outline of the main service provider of the whole process management and the entry pool of the questionnaire for patients with inflammatory bowel disease through literature research.Based on the semi-structured interview with the main providers of the whole process management service and the questionnaire survey on the patients with inflammatory bowel disease,the operation process and main service contents of the whole process management model for the patients with inflammatory bowel disease based on the cloud platform were preliminarily formulated,and the preliminary concept of the whole process management cloud platform framework structure and main functions was formed.Through the expert meeting method,the operation process,service content,platform framework structure and functional modules of the preliminary whole process management model were further revised and improved,and the whole process management model for patients with inflammatory bowel disease based on the cloud platform was formally constructed,and the whole process management cloud platform was designed and developed.[Results] 1.Semi structured interview results: the interviewed inflammatory bowel disease doctors,nurses,nutritionists,medical administrators and nursing management experts have a supportive attitude towards the whole process management of patients with inflammatory bowel disease and the use of internet technology to build the whole process management model of inflammatory bowel disease based on cloud platform,and are willing to participate in the model construction and provide corresponding disease management services.By summarizing the semi-structured interview data,the operation process and main service contents of the whole process management model of inflammatory bowel disease based on cloud platform from the perspective of the main providers of the whole process management service are extracted.2.Results of the survey: Among the 612 patients with IBD,57.68% of them had heard of the Internet plus whole process management service,45.75% of them used,92.16% of them had intention to use;When the education level,the monthly income and the diagnosis were different of the patients,there was a statistically significant difference in their intention to use the Internet plus whole process management service(P <0.05);The demand rates of the management services listed in the questionnaire were all >75%.The top five items are feedback of abnormal results,disease feedback and guidance,online inspection results,electronic health education,appointment diagnosis and treatment,with a demand rate of more than 90%.3.The formal construction results of the model: Based on the cloud platform,the three elements of the whole process management model for patients with Inflammatory bowel disease are cloud platform,patients and management team,in which patients are the core of the model,and cloud platform is the medium for the operation of the model.The three elements are connected by the patient terminal and medical terminal through the Internet,forming a closed loop.Based on the patient's health records,the model links up the links before,during and after diagnosis,and establishes a closed-loop service integrating screening,triage,treatment,follow-up and self-management.Model operation process: patients are managed at home by the terminal,symptom monitoring and self-evaluation.The cloud platform analyzes the health data of patients,evaluates the health status of patients,and gives corresponding feedback.Patients and management team deal with it in a timely manner,re-monitor,re-evaluate and re-intervene,and carry out closed-loop management of recycling.4.Results of cloud platform design and development: the whole process management cloud platform for patients with inflammatory bowel disease is set with patient side and medical care side,including three management channels: app,We Chat and web platform.The patient side provides services such as doctor-patient interaction,health consultation,self-management support,health data upload,test report data and information support acquisition,etc.The medical and nursing end supports interaction,automatic follow-up,remote monitoring and management,health intervention and other functions.[Conclusion] On the basis of literature research,semi-structured interviews and questionnaire survey,this study preliminarily determined the operation process and main service content of the whole process management model for patients with inflammatory bowel disease based on cloud platform,and formed the preliminary concept of the whole process management cloud platform framework structure and main functions.After the revision through expert meeting,this study constructed the whole process management model,designed and developed the whole process management cloud platform for patients with inflammatory bowel disease.It provided a direction for the management of inflammatory bowel disease with high efficiency,convenience and intelligence,and provided theoretical basis,methodological reference for the construction and exploration of the Internet plus chronic disease management model.
