Effect Of Case Management Based On Internet Platform In Patients With Stable COPD

Research Purpose:Based on the case management model and using self-efficacy theory,we implemented an Internet-based case management platform for COPD patients.To explore the influence of Internetbased medication management platform(CAT)on the quality of life and self-efficacy of patients with COPD.Research Methods:A randomized controlled study design was used in this study.Ninety-two COPD patients who met the inclusion and exclusion criteria and were hospitalized in the respiratory department of a tertiary care hospital in Huzhou City between January 2021 and December 2021 were used as the study population,and the 92 patients were grouped using a randomized controlled study method,and grouped as follows:The random numbers generated from the random number table were entered into the number,and then the 92 random numbers were numbered in order from smallest to largest,and we specified 1-46 as the control group and 47-92 as the intervention group.In the control group,routine care and follow-up were provided;in the intervention group,in addition to routine care and follow-up,an Internet-based case management intervention was implemented,in which a personalized management plan was developed for the patients according to their priority needs,and patients were provided with real-time answers to their questions and encouraged to share their experiences on the Internet platform for a total of 3 months.SPSS 26.0 statistical software was applied to statistically analyze the data,and statistical methods included descriptive statistical analysis,t-test,and Fisher’s exact test with test level α=0.05.Research Results:1.Loss of study subjects: 46 cases were included in each of the two groups before the intervention,and a total of 5 cases were lost.Among them,2 patients in the intervention group withdrew from the study,1 of them withdrew from the study because they felt that the study process was complicated,and 1 patient was removed because he had no interaction with the study team for a long time;3 cases in the control group withdrew from the study,2 cases had wrong telephone calls,and 1 patient died and withdrew from the study.Thus,at the end of the study,a total sample size of 87 cases was collected,44 in the intervention group and 43 in the control group.2.Result analysis:(1)There were no statistically significant differences between the two groups when comparing general information such as age,gender,marital status,education,occupation,per capita monthly income,as well as self-efficacy,self-management ability,quality of life(CAT),and medication adherence(P>0.05).(2)After 1 month and 3 months of intervention,the self-efficacy,self-management ability,quality of life(CAT)and medication compliance of patients in both groups improved,and the difference was statistically significant(P<0.05).A repeated-measures ANOVA found that the selfefficacy,self-management ability,and quality of life(CAT)scores of the two groups of patients had inter-group,intra-group,and interaction effects at different time points,and a further two-by-two comparison of the two groups of patients at different time points showed that the self-efficacy,selfmanagement ability,and quality of life(CAT)scores of the patients in the intervention group increased more than those in the control group,with a statistically significant difference(P<0.05).The difference was statistically significant(P<0.05).(3)Number of patient readmissions and readmissions: After 1 month and 3 months of intervention,the differences in the number of readmissions and readmissions between the two groups were not statistically significant(P>0.05).
