Objective:To observe the effect of silver needle on the expression of5-hydroxytryptamine(5-HT)and 5-hydroxytryptamine 2A(5-HT2A)receptor in the central nervous system of myofascial pain syndrome(MPS)rats,and to explore the central mechanism of silver needle therapy for MPS.Methods: 54 rats were randomly divided into 3 groups,18 rats in each group.The normal group was not stimulated.The MPS model was established in the model group by the way of strike combined with forced movement.The silver needle group was treated with heat conduction by silver needle on the basis of successful model.The Pathological section of the right medial femoral muscle,paw withdrawal thermal letency(PWTL),electromyogram(EMG),5-HT and 5-HT2 A receptors of medulla oblongata and spinal cord were measured on the 1st,7th and 14 th day after the treatment.Results:(1)PWTL: The PWTL of the model group and the silver needle group was significantly shorter than that of the normal group at each time point(P<0.05);the PWTL of the silver needle group was not significantly changed after the treatment for 1 day(P> 0.05);PWTL significantly increased and showed an increasing trend day after day after treatment,and the difference was statistically significant(P <0.01).(2)Electromyography: After the model,the rat's right medial femoral muscles showed frequent spontaneous power generation activities.After silver needle treatment,the spontaneous power generation activities gradually decreased,and sporadic muscle spontaneous power generation activities were still visible 14 days after treatment.(3)The pathological section of the right medial femoralmuscle: the normal group had regular morphology and neatly arranged muscle fibers;the model group had interstitial hyperplasia and disordered arrangement of muscle fibers;1 day after the silver needle treatment,there was no significant difference compared with the model group;7 days after the silver needle group treatment The muscle fiber morphology gradually recovered and the interstitial hyperplasia improved better than before;14 days after treatment in the silver needle group,the muscle fiber morphology was more regular and neovascularization was visible.(4)Central 5-HT positive expression: 5-HT in the medulla oblongata in the model group was lower than that in the normal group,the difference was statistically significant(P <0.01);the 5-HT expression in the silver needle group was up-regulated at each time point,and showed an increasing trend,the difference was Statistical significance(P <0.01).The expression of 5-HT in the spinal cord of the model group decreased compared with the normal group.There was no significant change in the level of 5-HT 1 day after the silver needle group treatment.The expression of 5-HT gradually increased at 7 and 14 days after the silver needle treatment.The point difference is statistically significant(P <0.01).(5)Spinal cord 5-HT2 A receptor: There was no significant difference between the normal group and the model group at each time point;the silver needle group increased the expression of 5-HT2 A receptor at each time point compared with the normal group and the model group;7 days after treatment,The expression of 5-HT2 A receptor was significantly increased and showed an increasing trend at 14 days compared with 1 day after treatment.The difference was statistically significant(P <0.05).Conclusion: 1.The levels of 5-HT in the medulla oblongata and spinal cord of the MPS model rats decreased,and there was still a tendency to decrease over time;2.After the silver needle treatment,the5-HT levels in the medulla oblongata and spinal cord of MPS rats increased significantly compared with normal rats,and over time There was an increasing trend,in which the medulla oblongata changed earlier than the spinal cord;3.The positive expression of 5-HT2 A receptor in the spinal cord of MPS rats after silver needle treatment gradually increased with the time after treatment and increased with 5-HT,thermal hyperalgesia recovery,myoelectricity Figure recovery and local medial femoral muscle repair showed a synchronous trend.It is speculated that silver needle thermal therapy may exert its analgesic effect by regulating the central 5-HT and5-HT2 A receptors.
