ObjectiveTo compare the short-term and long-term efficacy and safety of comprehensive treatment, silver needle combined with anatomic train and myofascial and pure silver needle heating in treatment of low, moderate, severe KOA patients.MethodA total of 300 patients with knee osteoarthritis who met the diagnostic criteria from August 2015 to January 2017 were enrolled. They were divided into comprehensive treatment group(102 cases),traditional intensive silver needle therapy group(98 cases)and fine silver needle therapy group(100 cases). The comprehensive treatment group with silver needle heat therapy combined with sodium hyaluronate injection and anatomic train and myofascial treatment method combined treatment, traditional intensive silver needle therapy group treated by silver needle heat therapy combined with anatomic train and myofascial, fine silver needle therapy group with silver needle therapy in the treatment of heating. The 3 groups were treated for 1 years. The VAS score,WOMAC score, clinical efficacy and safety evaluation index of 3 groups were investigated 2 weeks,1 months,3 months,6 months and 12 months,respectively.Results1.Comprehensive therapy group, mild to moderate or early, middle and late KOA were treated for 6 months and 12 months VAS score and WOMAC score was significantly lower than the corresponding standard pure silver needle treatment group and treatment of silver needle combined with anatomic train and myofascial group, mild to moderate or early middle KOA 3 months of treatment in patients with VAS score significantly the standard is lower than the corresponding pure silver needle treatment group and treatment of silver needle combined with anatomic train and myofascial group, severe or advanced KOA patients 3 months of treatment,VAS score was significantly lower than that of severe or advanced pure silver needle treatment group, mild or early KOA patients 1 months VAS score significantly lower than mild or early pure silver needle treatment group and treatment of silver acupuncture combined with anatomic train and myofascial group, moderate and severe,or in patients with advanced KOA treatment for 1 months,VAS score was significantly lower than that of the corresponding standard pure silver needle treatment group, mild to moderate or early middle KOA patients For 2 weeks, the score of VAS was significantly lower than that of the simple silver needle group with the corresponding standard(P < 0.05).2.There was no significant difference in the total effective rate of short-term clinical efficacy between the 3 groups of light or moderate(P > 0.05).The total effective rate of severe or late short-term clinical treatment in the comprehensive treatment group was higher than that in the fine silver needle therapy group, the difference was statistically significant(all P < 0.05).3.3 groups of mild, moderate or severe KOA patients had no serious adverse reactions during the treatment for 12 months. The safety evaluation of KOA patients in the 3 groups was not statistically significant(P > 0.05).Conclusion:1.The short-term and long-term effect of the comprehensive therapy and traditional intensive silver needle therapy are better than the fine silver needle therapy; The short-term effect of the comprehensive therapy is better than the traditional intensive silver needle therapy group.2.There was no serious adverse reaction among comprehensive therapy and traditional intensive silver needle therapy group and fine silver needle therapy group, and the safety was comparable among tree groups.3.The disease degree of patients divided by the staging standard could guide the treatment of patients during the treatment for 12 months. The safety evaluation of KOA patients in the 3 groups was not statistically significant(P > 0.05).
