Objective:To evaluate the effect and significance of 3D laparoscopy in rectal cancer surgery.Method:We manually searched the published literatures about the comparison of 3D laparoscopy and traditional 2D laparoscopy in rectal cancer surgery in major databases at home and abroad.Two graduate students strictly established inclusion and exclusion criteria,screened the literature and used Cochrane Risk of Tool to evaluate the methodological quality,and extracted the data independently.The stability of the result is checked by sensitivity analysis,and the conclusion is drawn after data processing and analysis by using ReviewManger5.3 provided by Cochrane collaboration network.Result:After repeatedly using the method of combining subject words with free words and other retrieval methods,185 related literatures were retrieved for the first time.According to the entry criteria,a total of 649 patients were included in 8 studies after reading the title,abstract and full text in detail.This study suggests that compared with traditional 2D laparoscopy,radical resection of rectal cancer with 3D laparoscopy is significantly less in operation time(MD=-17.52,95%CI(-22.16,-12.88),P < 0.00001),intraoperative blood loss(MD=-19.53,95%CI(-30.22,-8.85),P=0.0003),postoperative hospital stay(MD=-0.41,95%CI(-0.67,-0.16,P=0.001)and the incidence of postoperative complications(OR=0.55,95%CI(0.34,0.87,P=0.01),but there is no statistical significance in the number of lymph node dissection(MD=1.27,95%CI(-0.45,2.98),P=0.15)and postoperative exhaust time(MD=-0.40,95%CI(-0.81,-0.01),P=0.06).Conclusion:This study shows that 3D laparoscopic radical resection of rectal cancer canreduce the operation time,intraoperative blood loss,postoperative hospital stay and the incidence of complications when compared with 2D laparoscopy,and has obvious advantages in shortening the operation time.It is a modern imaging surgery system worthy of implementation.Due to the small number of samples and low quality of literature,the conclusions of this study still need to be supported by multicenter large-sample randomized controlled trials.
