Objective: This paper evaluates systematically the risk factors of anastomotic leakage after laparoscopic resection of rectal cancer in recent ten years with the way of a meta-analysis,and to provide decision-making for clinical treatment,which could provide better decision for clinical treatment.Methods: According to the inclusion and exclusion criteria,the related literatures on the analysis of the risk factors of anastomotic leakage after laparoscopic resection of rectal cancer were searched in the database(Pub Med,Embase,The Cochrane Library)and the Chinese database(CNKI,Wan fang Database).The time of searching is set from January 2007 to January 2017.We filter the high quality of the literature conformed to the inclusion criteria and extract the data after browsing the subject,abstract and full text.The search time is set from January 2007 to January 2017,after browsing the subject,abstract and full text,screen out in line with the inclusion,extract the data.The meta-analysis was performed by Review Manager 5.3.Results: A total of 19 meta-analyzes were included in this pape.The total sample size was 8422 cases,495 cases appear postoperative anastomotic leakage and the incidence of postoperative anastomotic leakage is 5.88% approximately.The result of meta-analysis showed that:sex [OR=2.41,95%CI(1.93-3.00),P <0.00001];preoperative albumin [OR=0.59,95%CI(0.38-0.90),P =0.01];Diabetes [OR=1.48,95% CI(1.06-2.07),P=0.02];preoperative BMI [OR=1.64,95%CI(1.01-2.65),P=0.005];preoperative neoadjuvant chemoradiotherapy [OR = 1.73,95% CI(1.27-2.36),P = 0.0005];the distance of tumor from the anal margin [OR=0.60,95%CI(0.46-0.77),P<0.0001];tumor size [OR = 2.10,95% CI(1.35-3.27),P = 0.001].Conclusion: Male,preoperative albumin <3.5mg/d L,preoperative diabetes mellitus,BMI? 25kg/m2,preoperative neoadjuvant chemotherapy,the distance of anastomosis from the anal margin? 7cm and tumor diameter? 5cm are major risk factors of anastomotic leakage after laparoscopic resection of rectal cancer.
