Objective:To explore and analyze the risk factors of hemorrhagic stroke in adult patients with moyamoya disease,and to provide some evaluation methods and prevention measures for the prevention of hemorrhagic stroke in moyamoya disease patients.Methods:Retrospective analysis of 275 patients diagnosed as mmd by DSA or MRA,CTA from June 2017 to December 2018 in neurosurgery,Second Affiliated Hospital of Nanchang University,including a total of 135 cases of hemorrhagic stroke moyamoya disease(study group),including 59 cases of moyamoya disease without stroke(control group),excluding ischemic stroke cases 79 cases,minor patients 2cases;further collect specific clinical relevant data,including sex,age,BMI,smoking history,drinking history,blood pressure,blood glucose,monobilateral moyamoya disease,moyamoya disease stage,whether complicated with aneurysm,family history of intracerebral hemorrhage,family history of moyamoya disease,etc.;and all the data were statistically analyzed to obtain the associated risk factors of hemorrhagic stroke in adult moyamoya disease.Rresults:1.univariate analysis: smoking(P=0.021),hypertension(P=0.028),family history of cerebral hemorrhage(P=0.049),family history of moyamoya disease(P=0.042),moyamoya disease stage(P=0.000),aneurysm(P=0.032)were statistically different between the two groups(p<0.05);There were no statistically significant differences between the two groups(p>0.05)in gender(P=0.517),age(P=0.862),obesity(P=0.654),alcohol consumption(P=0.816),diabetes(P=0.448)and moyamoya disease(P=0.510).2.Multivariate analysis: smoking(P=0.012),moyamoya disease stage(P=0.000),and aneurysm(P=0.021)were statistically different between the two groups(p<0.05)There were no statistically significant differences in family histories of hypertension(P=0.053),cerebral hemorrhage(P=0.150)and moyamoya disease(P=0.999)between the two groups(p>0.05).Conclusions:1.The risk factors for hemorrhagic stroke in moyamoya disease patients are smoking,moyamoya disease stage and aneurysm,while sex,age,obesity,alcohol consumption,hypertension,diabetes,moyamoya disease single and bilateral,cerebral hemorrhage and family history of moyamoya disease are not the risk factors for their occurrence of hemorrhagic stroke;2.Moyamoya disease patients with Suzuki Phase III IV may be more prone to hemorrhagic stroke.
