Objectives:Preliminary study on the expression of CCR3 in eosinophilic CRSw NP and the role of related Th1/Th2 cytokines.To observe the role of CCR3 and related Th1/ Th2 cytokines in the pathogenesis of eosinophilic CRSw NP.Methods:30 patients diagnosed as CRSw NP in our hospital were stained with HE,15 patients with eosinophils < 10% infiltrating inflammatory cells were included in the non-eosinophilic CRSw NP group,15 patients with eosinophils ?10% infiltrating inflammatory cells were included in the eosinophilic CRSw NP group,and 10 patients diagnosed with nasal septum deviation and undergoing nasal septum correction surgery in our hospital were selected as the normal Control group.The nasal polyp tissue of CRSw NP patients and the inferior turbinate mucosa of the Control group were collected.The expression of CCR3,Eotaxin,and ECP in nasal polyps and inferior turbinate mucosa were detected by immunohistochemical staining,and the concentration changes of CCR3?Eotaxin?ECP?Th1(TNF-??IFN-? ? IL-12)and Th2(IL-4 ? IL-5 ? IL-13)cytokines were determined by using commercial ELISA kits.In addition,nasal polyps from 3 patients with eosinophilic CRSw NP(ECRSw NP)and 3 patients with non-eosinophilic CRSw NP(NECRSw NP)cultured in vitro for 24 hours were divided into 3 groups separately.Group A: No stimulant was added to the original medium;Group B: Add the CCR3 agonist(Eotaxin)10ng/m L,100ng/m L,1000ng/m L separately;Group C: Add the CCR3 inhibitor(SB-328437)10n M,100 n M,1000 n M separately.The expression of ECP ?Th1(TNF-??IFN-??IL-12)and Th2(IL-4?IL-5?IL-13)cytokines in the culture medium were detected by commercial ELISA kits.Results:H&E staining showed that a large number of inflammatory cells dominated by eosinophils were found in ECRSw NP group,while in NECRSw NP group,the main inflammatory cells were lymphocytes?plasma cells or neutrophils;in the Control group,a small number of lymphocytes and plasma cells were seen,and eosinophils were seldomly seen.Immunohistochemical staining showed that the expression of CCR3?Eotaxin and ECP were significantly increased in the ECRSw NP group,and were statistically significant compared with the Control group and NECRSw NP group(P <0.05);the expression of CCR3 and ECP were significantly higher in NECRSw NP group than Control group(P<0.05).Elisa analysis in each group showed:(1)Compared with the Control group and the NECRSw NP group,the expression of Eotaxin and ECP were significantly increased in the ECRSw NP group(P<0.05),but there was no significant difference between the NECRSw NP group and the Control group(P>0.05).(2)Compared with the Control group,the expression of CCR3 was significantly increased in the ECRSw NP group(P<0.05),while compared with the NECRSw NP group,the expression of CCR3 was increased in the ECRSw NP group without statistical significance(P>0.05).(3)Compared with the ECRSw NP group and the Control group,the expression of IFN-? was significantly increased in the NECRSw NP group(P<0.05).(4)Compared with the Control group,the expression of IL-13 was significantly increased in the ECRSw NP group and NECRSw NP group(P<0.05),but there was no significant difference between the ECRSw NP group and NECRSw NP group(P>0.05).(5)Compared with the NECRSw NP group and the Control group,the expression of TNF-? ? IL-4 and IL-5 were significantly increased in the ECRSw NP group(P<0.05),but there was no significant difference between the NECRSw NP group and the Control group(P>0.05).In the vitro culture of nasal polyps,Elisa analysis showed that there was no significant difference in the expression level of ECP?IFN-??TNF-??IL-4?IL-5?IL-12 and IL-13 among groups A ? B and C under the effects of different concentrations of CCR3 agonists and inhibitors.Conclusion:The expression of CCR3 was significantly increased in patients with ECRSw NP,suggesting that CCR3 play a role in the pathogenesis of ECRSw NP.Th1/Th2 mixed inflammation was dominanted in ECRSw NP patients,while Th1 inflammation was dominanted in NECRSw NP patients.In vitro culture results of nasal polyp tissues showed that the regulation of CCR3 did not affect the expression of Th1/Th2 cytokines,indicating that CCR3 may not play a role in the pathogenesis of ECRSw NP by regulating Th1/Th2 cytokines.
