The Clinical Observation Of Recurrent Spontaneous Abortion Caused By The Treatment Of Prethrombus Status By Bushen Huayu Qushi Method

Objective:In this study,we observed the clinical effect of Bushen Huayu Qushi Method on RSA caused by prethrombotic state,and evaluated the therapeutic effect and mechanism of Huayu Qushi Decoction and Bushen Zhongyu Decoction.By comparing the success rate of pregnancy and the effective rate of TCM syndrome,and the changes of TCM syndrome score,single symptom score and related laboratory indexes(coagulation four items,D-dimer,anticardiolipin antibody)before and after treatment,so as to explore a safe and effective new way for TCM diagnosis and treatment of RSA prethrombotic state.Method: Sixty patients with pre-thrombotic state of RSA conforming to kidney deficiency,blood stasis and damp resistance were randomly divided into treatment group and control group,30 cases in each group.Huayu Qushi Decoction was taken orally from 1 to 5days of menstruation,Bushen Zhongyu Decoction was taken from the 6th day of menstruation.Meanwhile,aspirin enteric-coated tablets were taken orally.The control group simply took aspirin enteric-coated tablets,and both groups were treated for 3 menstrual cycles.To observe the improvement of four items of coagulation,D-dimer and ACA antibody before and after treatment,and the difference of TCM syndrome scores and pregnancy success rate.Result:1.There were no statistically significant differences in age,number of spontaneous abortions,degree of illness,single TCM symptom score,total score of TCM syndromes,and four items of coagulation,D-dimer,ACA antibody positive rate before treatment between the two groups(P>0.05).2.After treatment,the success rate of pregnancy preservation was 69.23% and 85.71%,respectively.The rate in the treatment group was better than the control group,and the difference was statistically significant(P<05 0.).3.The effective rates of TCM syndromes of the two groups after treatment were 76.67%and 90.32%,respectively.The effective rates in the treatment group were significantly much higher than the control group(P<0.01).The total score of TCM syndromes after treatment in both groups was lower than that before treatment(P<05 0.),the decrease of the treatmentgroup was better than the control group(P<05 0.).All the scores of single TCM symptoms were lower than those before treatment,except for abdominal fullness and distention of the control group,the differences within the groups were statistically significant(P<05 0.).The single symptom improvement in the treatment group was better,except for dizziness,tinnitus and dull complexion,the difference between the groups was statistically significant(P<05 0.).4.After treatment,the levels of APTT,PT,and TT in the two groups were higher than before,except for the PT of the control group,the differences within the groups were statistically significant(P<05 0.).The rise of APTT,PT and TT in the treatment group was better than the control group,except for TT,the differences between the groups were statistically significant(P<05 0.).After treatment,FIB in the two groups was lower than that before treatment(P<05 0.),there was no significant difference between the two groups after treatment(P>0.05).5.After treatment,the level of D-dimer in both groups was lower than before(P<05 0.),the improvement of D-dimer in the treatment group was better than the control group(P<05 0.).6.After treatment,the negative conversion rate of ACA antibody in the two groups was44.00% and 80.77%,respectively.The rate in the treatment group was better than that control group(P<05 0.).Conclusion:The therapeutic effect of Bushen Huayu Qushi combined with western medicine on prethrombotic state of RSA with kidney deficiency,blood stasis and dampness obstruction is significant.After the treatment,the success rate of pregnancy and the effective rate of TCM syndrome in the two groups were better than Western medicine alone.The coagulation index and D-dimer level of the patients were improved,and the negative rate of anticardiolipin antibody was increased.It showed that the treatment of invigorating the kidney and removing blood stasis and removing dampness combined with aspirin could regulate the hypercoagulable state of blood,regulate the autoimmunity,improve the adverse pregnancy outcome,and there was no obvious side effect,safe and reliable.
