Research purposes:Coronary heart disease(CHD)causes financial burden and death.Particularly,myocardial infarction worses rapidly and has high mortality.The gold standard for the diagnosis of coronary heart disease mainly relies on the invasive operation of coronary angiography.Many patients have poor compliance,resulting in untimely treatment.The relationship between CHD and non-invasive examination of peripheral blood inflammation markers has become a research hotspot,but they are all in the preliminary stage of research.The relationship between LncRNA and disease has become hot research topic today.Smooth muscle induced-lncRNA enhances replication(SMILR)is an LncRNA found in vitro and related to inflammation and smooth muscle cell proliferation.Studies had shown that lncRNA SMILR played an important role in the unstable state of atherosclerotic plaques,but its relationship with coronary heart disease has not been reported so far.This study intends to detect the relative expression of LncRNA SMILR in peripheral blood of patients with CHD and non-coronary heart disease by quantitative real-time polymerase chain reaction(qRT-PCR)technology,comparing the difference of SMILR transcription level between the two groups,Analyze its correlation with hypersensitive c-reactive protein(hs-CRP),interleukin-6(IL-6),and neutrophile lymphocyte rate(NLR).The ability of peripheral blood LncRNA SMILR content to predict the occurrence of coronary heart disease is evaluated.OBJECT AND METHODS:We selected the 117 patients who underwent coronary angiography from January 2019 to January 2020,included 60 cases in the coronary heart disease groups,and 57 cases in the non-coronary heart disease group.All patients recorded age,gender,hypertension,diabetes,smoking history,etc.The indexes of hs-CRP,IL-6,NLR and blood lipid were detected respectively.LncRNA SMILR expression was detected by qRT-PCR.Statistical comparison:1.The relationship between LncRNA SMILR and CHD.2.Correlation between SMILR and various inflammation indexesin CHD group.3.Compared the correlation between Gensini score and LncRNA SMILR expression,IL-6,hs-CRP,NLR in CHD group,and compared the expression of LncRNA SMILR in high and low risk groups.4.analyzed the relationship between LncRNA SMILR and relevant clinical features of patients with coronary heart disease.5.Made ROC curve to calculate the sensitivity and specificity of LncRNA SMILR,IL-6,hs-CRP,NLR for the diagnosis of coronary heart disease,.6.Used logistic regression to analyze the risk factors of patients with coronary heart disease.7.All data were using SPSS23.0 for Windows statistical data,P <0.05(two-sided)difference is statistically significant.RESULTS:1.General situation analysis : Between two groups,gender,age,diabetes,proportion of smoking patients,TC,TG,LDL-C levels were not statistically significant(P> 0.05),HDL-C,hypertension levels were significantly different(P<0.05).2.Difference in LncRNA SMILR expression and related inflammation indexes:Compared with non-coronary heart disease group,the expression of LncRNA SMILR,IL-6 and hs-CRP in CHD patients with coronary heart disease were significantly increased(P <0.05).3.Correlation analysis of LncRNA SMILR expression and related inflammation indexes:LncRNA SMILR expression level was positively correlated with hs-CRP、NLR、IL-6(P <0.05).4.Correlation analysis of Gensini score and LncRNA SMILR 、 related inflammation indexes : Gensini score was positively correlated with hs-CRP and SMILR(P <0.05),unfortunately,was not significantly correlated with IL-6 and NLR(P > 0.05).High-risk group SMILR has significantly higher expression than the low-risk group(P<0.05).5.Correlation between LncRNA SMILR and related clinical indicators:SMILR had no obvious correlation with gender,age,blood lipids,smoking,hypertension,and diabetes in patients with coronary heart disease(P > 0.05),and the coronary heart disease group was grouped according to gender,history of hypertension,diabetes,smoking,there was no significant difference in the expression of SMILR in eachgroup(P>0.05).6.Analysis of ROC curve of LncRNA SMILR and related inflammation indexes:ROC curve of coronary heart disease,the cut-off value of SMILR was 2.31 and AUC was 0.701;the cut-off value of hs-CRP was 5.35mg/L and AUC was 0.671;the cut-off value of IL-6 was 5.79pg/ml and AUC was0.647.the cut-off value of NLRwas 3.18 and AUC was 0.599.7.Analysis of risk factors of coronary heart disease:After assignment,through single factor logstic regression analysis,it was found that hypertension,high-density lipoprotein cholesterol,hs-CRP,IL-6,NLR,SMILR were risk factors for coronary heart disease,Multivariate regression analysis found that hypertension,high-density lipoprotein cholesterol,SMILR,and hs-CRP were independent risk factors for coronary heart disease(P <0.05).Conclusions1.LncRNA SMILR expression increased,and has certain clinical reference value for the diagnosis of coronary heart disease,the value has more advantages than other inflammatory markers.2.LncRNA SMILR has an independent correlation between coronary heart disease.So it can diagnose coronary heart disease independently.3.LncRNA SMILR is related to severity of coronary artery stenosis,because the high-risk group has high expression.
