| Objective: With the continuous advancement of China’s medical and health system reform,the effectiveness of the reform has continued to expand,but the relationship between doctors and patients has become increasingly tense and even the swords are facing each other,and the number of medical disputes continues to rise.If you are slightly dissatisfied,you will insult the medical staff,even malicious beatings and slashing.This kind of bad crisis has occurred in every city in the country.Severe and serious incidents that lead to the death of doctors have also occurred frequently,seriously affecting the normal hospital order,and causing tremendous mental stress to medical staff.Although the state and local governments have issued a number of documents to suppress the incidence of malignant injuries,medical disputes are still performed in hospitals every day.It’s a squabble,but it’s a plus,and even a full-fledged martial arts.Therefore,how to deal with the medical disputes that have been generated scientifically and properly in the event of medical disputes,to avoid escalating disputes into crisis events,and to minimize the adverse effects and harms caused by disputes is a major problem facing public hospitals.Methods: From the perspective of crisis management,this paper summarizes and analyzes the relevant theoretical research and practical results of crisis management and medical disputes at home and abroad,and combines the actual situation of the hospital to build a scientific and reasonable and effective medical dispute security system based on the classic 4R model.Effectively handle and control the whole process of medical disputes and development;take Shanghai J Hospital as the specific research object,and collect statistics on the key indicators such as the incidence of medical disputes and the amount of medical disputes before and after the application of the medical dispute security mechanism.To analyze and analyze the medical disputes,prevention,control and protection of medical staff in the hospital through self-designed questionnaires,and then analyze the typical medical disputes cases,and construct them from quantitative and qualitative aspects.The effectiveness of the medical dispute security mechanism is evaluated.Results: 150 questionnaires were distributed and 144 questionnaires were returned.Statistical analysis of the questionnaire data was performed by statistical software SPSS 20.0.According to the survey results,47.92% of medical staff believe that the current doctor-patient relationship is tense,and the proportion of people who are very nervous is 23.61%.Only 2% of medical staff are more optimistic about the current doctor-patient relationship.In the survey,60.41% and 26.39% of the medical staff respectively believed that the safety of medical disputes was most effective in the prevention and early warning stages.The results of the investigation and the medical dispute safety protection mechanism constructed in this paper are mainly prevention and prevention.The principle of the past is consistent,indicating that the protection of medical dispute crisis management is focused on prevention.Establish an ex-ante group and an ex-post group.From 2014 to 2015,rely on traditional experience to deal with medical disputes.This stage is called the ex-ante group.From 2016 to 2017,the hospital carried out crisis management research and established and applied the medical dispute security system.The prevention and control of medical disputes is called the ex-post group.In the ex-ante group from 2014 to 2015,the total number of hospitalized patients was 120,757,and there were 245 medical disputes,and the medical dispute rate was 2.029‰.In the ex-post group,the total number of hospitalizations was 210,849 during the period from 2016 to 2017.A total of 190 medical disputes occurred,and the medical dispute rate was 0.901‰.The incidence rate of ex-post group disputes was 0.901‰,which was significantly lower than the ex-ante incidence rate of 2.029‰.Through analysis,it can be concluded that the practical application of medical dispute security mechanism has a certain inhibitory effect on disputes.The medical dispute compensation amount data of the ex-post group and the ex-ante group were compared.In the ex-ante from 2014 to 2015,the total amount of medical disputes was 4.89 million,and the annual average compensation amount was 246.39±21.439 million.In the ex-post group from 2016 to 2017,the total amount of medical disputes was 3.29 million,and the annual compensation amount was 164.76±65.60 million.Conclusion: The research results show that the crisis management-based medical dispute security mechanism constructed in this paper can effectively control the probability of medical disputes escalating into medical conflict crisis events,while minimizing the adverse effects and losses caused by medical disputes as much as possible.It is of great theoretical and practical significance to alleviate the current increasingly tense doctor-patient relationship,safeguard the legitimate rights and interests of both doctors and patients,guarantee the normal operation order of medical institutions,and finally construct an equal,harmonious and respectful doctor-patient relationship. |
