Background and Objective: Adipokine adipsin contributes to insulin resistance(IR),which was thought to be a risk factor of mild cognitive impairment(MCI)in type 2 diabetes mellitus(T2DM).This study aimed to investigate the role of adipsin in Chinese T2 DM patients with early cognitive impairment and whether adipsin contributes to diabetic MCI caused by IR.Methods: A total of 126 hospitalized Chinese T2 DM patients were recruited and divided into two groups according to the Montreal Cognitive Assessment(MoCA)score.Demographic characteristics were collected, and neuropsychological tests were evaluated.Plasma level of adipsin was measured by Enzyme-linked Immunosorbent Assay. Results: T2 DM patients with MCI(n =57)exhibited a significantly increased plasma level of adipsin compared with their healthy controls(p =0.018).Among MCI patients, plasma adipsin level was negatively correlated with the MoCA,MMSE and VFT scores(r=-0.640,p <0.001;r=-0.612,p <0.001;r=-0.288,p =0.035;respectively).Correlation analysis also demonstrated that adipsin level was positively correlatedwith fasting C-peptide;homeostasis model of assessment for insulin resistance;body mass index(r=0.368,p<0.001;r=0.494,p <0.001;r=0.336,p =0.011;respectively).Multivariable regression model further indicated that adipsin was an independent risk factor of MCI in T2 DM patients(p=0.017).Conclusions: Elevated plasma level of adipsin was associated with MCI in Chinese T2 DM patients.Further studies should be conducted to determine whether adipsin was involved in IRrelated susceptibility to early cognitive decline in T2 DM patients.
