Objective to discuss the effect of pioglitazone in type 2 diabetic patients with mild cognitive impairment clinical efficacy and evaluation methods.Methods The patients clinical staging,the use of cognitive assessment tools screening of type 2 diabetes mellitus(T2DM)patients with mild cognitive impairment(MCI)patients with 66 cases,given pioglitazone hydrochloride oral treatment on 15mg/in patients with basic therapy,treatment for 2 years.Before treatment,treatment for 1 years and 2 years after the detection of glycated hemoglobin(HbAlc),fasting insulin(FINS),fasting blood glucose(FBG),blood pressure,triglyceride(TG),cholesterol(TC),serum uric acid,insulin like growth factor-I(IGF-I),transforming growth factor beta 1(TGF-beta l),The insulin resistance index(HOMA-IR)was calculated,and the Montreal cognitive assessment(MoCA),the mini mental state examination(MMSE)and the event-related potential(ERP)P300 assessment were repeated 1 years before treatment and 2 years later.Results Pioglitazone treatment after 1 years of FBG,HbAlc,FINS,TG,HOMA-IR,TGF-beta 1 level drops,P300 latency and increased IGF-I level difference was statistically significant(P<0.05);blood pressure decreased after 2 years of treatment,MMSE score increased,the amplitude of P300 increased significantly(P<0.05);IGF-1,TGF-beta 1 the level of MoCA score,the latency of P300 treatment had significant difference(P<0.001).Conclusions(1)pioglitazone treatment can improve and slow the cognitive function of patients with type 2 diabetes loss,treatment needs for a long time,may be related to the improvement of insulin resistance.(2)the evaluation of mild cognitive impairment to choose the high sensitivity of the tool,P300 is better sensitive.
