Objective: To investigate the relationship between serum 25(OH)D level and atrial fibrillation in patients with hyperthyroidism and its clinical significance.Methods:A total of 288 hyperthyroidism patients,122 males and 166 females,who were hospitalized in the Affiliated Hospital of Zunyi Medical University from July2017 to July 2019,were retrospectively collected.According to the presence or absence of atrial fibrillation,hyperthyroidism patients were divided into hyperthyroidism with atrial fibrillation group(n=51 cases,25 males/26 females),hyperthyroidism without atrial fibrillation group(n=237 cases,97 males/140 females),and all enrolled patients were tested for serum 25-hydroxyvitamin D level.General data,various biochemical tests and clinical examination results of enrolled patients were collected.SPSS 17.0 statistical software was used for statistical analysis,multivariate logistic regression was used to analyze the relationship between each index and AF,and Pearson correlation analysis was used to analyze the correlation between 25(OH)D and other indicators.Results:1.There were significant differences in age,urea nitrogen,uric acid,lipoprotein a,platelet,calcium,phosphorus,magnesium,triglyceride,total cholesterol and TSH between the hyperthyroid patients with AF and the hyperthyroid patients without AF(P < 0.05).There was no significant difference in sex,leukocyte,neutrophil,lymphocyte,hemoglobin,creatinine,FT3 and FT4 between the two groups(P > 0.05).2.The serum level of 25(OH)D in the hyperthyroidism with atrial fibrillation group was significantly lower than that in the hyperthyroidism without atrial fibrillation group [(15.89±7.72)vs(19.31±9.03)],and the difference was statistically significant.3.Pearson correlation analysis showed that 25(OH)D levels in hyperthyroidismpatients were positively correlated with creatinine(r=0.195,P=0.001),urea nitrogen(r=0.120,P=0.043),calcium(r=0.187,P=0.001),but negatively correlated with lipoprotein a(r=-0.120,P=0.042).4.Multivariate logistic regression analysis showed that low 25(OH)D level was a risk factor for atrial fibrillation in hyperthyroidism patients(OR 0.947,95% CI:0.904-0.993,P=0.025).in addition,the results of this study also showed age(OR1.045,95% CI: 1.013-1.078,P=0.005),uric acid(OR 1.006,95% CI: 1.001-1.010,P=0.010),lipoprotein a(OR 1.003,95% CI: 1.001-1.005,P=0.010),lipoprotein a(OR1.003,95% CI: 1.001-1.005,P=0.P=0.008)were risk factors for atrial fibrillation in hyperthyroidism patients.Conclusion: The occurrence of AF in hyperthyroidism patients has a certain correlation with 25(OH)D level,and hyperthyroidism patients with low 25(OH)D level are more likely to develop AF.Low 25(OH)D is a risk factor for atrial fibrillation in patients with hyperthyroidism.
