Analysis Of Factors Affecting The Prognosis Of Patients With Hyperthyroidism Atrial Fibrillation

Background and Objective Hyperthyroidism heart disease is a clinically common complication of hyperthyroidism,which mainly due to excessive thyroid hormones entering the blood circulation to cause heart damage through a series of genomic and non-genomic effects on the heart.And the heart damage mainly manifests as arrhythmia,heart enlargement,heart failure and valvular heart disease,with atrial fibrillation as common arrhythmia.The studies have shown that about 10%-25% of patients with hyperthyroidism have atrial fibrillation,and about 55%-75% of atrial fibrillation spontaneously returns to sinus rhythm after hyperthyroidism recovery.Previous studies have focused on the mechanism of the occurrence of hyperthyroidism atrial fibrillation and analysis of its influencing factors,but there is a little analysis on the factors influencing the prognosis of atrial fibrillation.This study is to compare the clinical data of atrial fibrillation recovery group and atrial fibrillation persistence group in patients with hyperthyroidism atrial fibrillation and to explore the related factors affecting the prognosis of patients with hyperthyroidism atrial fibrillation.Methods The general information,laboratory data and cardiac color doppler examination results of patients with hyperthyroidism complicated with atrial fibrillation admitted to the First Affiliated Hospital of Zhengzhou University from January 2013 to December 2017 were collected.The thyroid function and electrocardiogram were regularly followed up by telephone and outpatient visits,and a total of 61 cases were included in the final analysis.According to whether the patient's atrial fibrillation was converted to sinus rhythm after half a year of normal thyroid function,the data were divided into two groups: atrial fibrillation persistent group(n=31)and atrial fibrillation recovery group(n=30).Using difference analysis and multivariate logistic regression analysis analyzed the data between the two groups.Results The right atrial area,left atrial dimensio,right ventricular diamete,left ventricular diameter,pulmonary artery pressure,atrial fibrillation course,and hyperthyroidism course in the atrial fibrillation recovery group were lower than those in the atrial fibrillation group(P<0.05).FT3,FT4,and diastolic blood pressure were higher than those in the atrial fibrillation group(P<0.05).Multivariate logistic regression analysis showed atrial fibrillation(OR: 0.983,95% CI: 0.97-0.996),left atrial diameter(OR: 0.818,95% CI: 0.71-0.943),FT4(OR: 1.051,95)%CI: 1.017-1.087)were independent related factors in the recovery of atrial fibrillation.Conclusions 1.The course of atrial fibrillation and left atrial diameter are the prognostic factors of hyperthyroidism atrial fibrillation.The longer the course of atrial fibrillation and the larger the left atrial diameter,the more difficult it is for hyperthyroid atrial fibrillation to restore sinus rhythm.2.High FT4 level indicates that hyperthyroidism atrial fibrillation is likely to restore sinus rhythm.
