| Objective: To investigate the effect of post abortion care(Post Abortion Care,hereinafter referred to as PAC)on the recovery and re-pregnancy of the uterus after abortion in the elderly pregnant women who have a plan of reproductive birth within one year.Methods:From November 1,2017 to February 1,2019,the elderly pregnant women(age ≥35 years)who were diagnosed as early pregnancy missed abortion and had a plan for reproduction within one year were selected.Random(random number table method)was divided into group A(PAC group)and group B(non-PAC group).A total of 160 cases were finally included in this study.A group performed PAC services on the basis of B group,and the B group received routine abortion education and observation after curettage.After 10 days of curettage and the second period of menstruation,both groups underwent transvaginal color Doppler ultrasound(mainly measuring the thickness of endometrium).The first menstruation was performed on an empty stomach in the morning of 3-5 days after curettage.The patients were followed up by telephone in January,March,June and December to find out the time of vaginal bleeding,menstrual relapse,endometrial thickness,menstrual recovery and re-pregnancy within one year.SPSS19.0 statistical software was used to analyze the data.Results:1.Age,Number of days of menopause,pregnancy,birth,education and other general clinical data of the two groups,there was no significant difference(P>0.05).2.The proportion of patients’ satisfaction was different between the two groups(Z=3.667,P<0.001),and the difference was statistically significant.The number of patients in the A group was 78,accounting for 97.5%,which was higher than that in the B group,64,accounting for 80.0%.3.The vaginal bleeding time of A group was(4.18±1.71)days shorter thanthat of B group(5.29±2.14)days,the difference was statistically significant(P<0.05).The menstrual relapse time of A group was(27.84±2.44)days shorter than that of B group(34.39±3.26)days,the difference was statistically significant(P<0.05).4.The endometrial thickness was(3.30 ± 0.66)mm at 10 days after curettage in A group,higher than that in B group(1.76±0.64)mm,the difference was statistically significant(P<0.05).Endometrial thickness in the second menstrual period of the two groups was not statistically significant(P>0.05).5.Compared with the previous menstrual volume,there were 2 cases in A group,the incidence rate was 2.5%.7 cases in group B had reduced menstrual flow,with an incidence of 8.8%,and the difference was not statistically significant(P>0.05).6.There was no re-pregnancy in A group within 3 months after operation,and in B group,there was one re-pregnancy within 3 months after curettage.The repregnancy rate was 52.5% in A group and 36.3% in B group,the difference was statistically significant(P<0.05).Conclusion: 1.The implementation of PAC service after abortion in elderly pregnant women can increase the satisfaction of patients.2.After the implementation of PAC services can promote the recovery of uterus after curettage,for the next pregnancy to prepare for pregnancy;3.The implementation of PAC service after early pregnancy delayed abortion for elderly pregnant women can improve the repregnancy rate within 1 year for elderly pregnant women with fertility requirements.It is worth the extensive promotion of the elderly pregnant women with fertility requirements after the occurrence of missed abortion. |
