Objective:To observe the efficacy and safety of the Centurion active-fluidics system during phacoemulsification combined with intraocular lens implantation.Methods:This prospective randomized controlled study comprised a total 90 eyes of 90 patients with age-related cataract admitted to the Second Hospital of Jilin University from December 2018 to Septemb er 2019.Patients with other medical history of the eye,history of eye surgery,and systemic diseases affecting visual acuity were excluded.They were randomly divided into two groups: study group(active-fluidics system)and control group(gravity-fluidics system).All patients underwent phacoemulsification combined with intraocular lens implantation by the same experienced doctor.The cumulative dissipated energy(CDE)and estimated fluid u sed(EFU)were recorded during the operation.Observe and compare corneal endothelial cell density(ECD),central corneal thickness(CCT),best corrected visual acuity(BCVA)and central foveal thickness(CFT)within the central 1mm before surgery and 1 week,1 month,3 months after surgery respectively.Results:1.There was statistically significant difference in the CDE between the study group and the control group during the operation(z=-2.110,p<0.05),but there was no statistically significant difference in the EFU between the study group and the control group(z=-1.639,p(29)0.05).2.There was no statistically significant difference in ECD between the study group and the control group at 1 week,1 month and 3 months after operation(t = 0.082,0.082,0.074;p> 0.05).In the study group,there were statistically significant differences in ECD at 1 week,1 month and 3 months after operation compared with pre-operation(z =-5.757,-5.841,-5.841;p<0.001).In the control group,there were statistically significant differences in ECD at 1 week,1 month and 3 months after operation compared with pre-operation(z =-5.841,-5.841,-5.841;p<0.001).3.There was statistically significant difference in CCT between the study group and the control group 1 week after surgery(t=-2.024,p<0.05).There was no statistically significant difference in CCT at 1 month and 3 months after operation(t=-1.286,-0.653;p(29)0.05).In the study group,there were statistically significant differences in CCT at 1 week,1 month and 3 months after operation compared with pre-operation(z =-5.512,-5.445,-4.764;p<0.001).In the control group,there were statistically significant differences in CCT at 1 week,1 month and 3 months after operation compared with pre-operation(z =-5.779,-5.714,-4.930;p<0.001).4.There was no statistically significant difference in BCVA(LogMAR)between the study group and the control group at 1 week,1 month and 3 months after operation(z =-0.688,0.098,0.832;p > 0.05).In the study group,there were statistically significant differences in BCVA(LogMAR)at 1 week,1 month and 3 months after operation compared with pre-operation(z =-5.843,-5.844,-5.844;p<0.001).In the control group,there were statistically significant differences in BCVA(LogMAR)at 1 week,1 month and 3 months after operation compared with pre-operation(z =-5.842,-5.843,-5.843;p<0.001).5.There was no statistically significant difference in CFT between the study group and the control group at 1 week,1 month and 3 months after operation(t =-0.442,-0.876,-0.345;p > 0.05).In the study group,there was no statistically significant difference in CFT at 1 week compared with pre-operation(z=-1.156,p(29)0.05).There were statistically significant differences in CFT at 1 month and 3 months after operation compared with pre-operation(z =-5.848,-5.312;p<0.001).In the control group,there were statistically significant differences in CFT at 1 week,1 month and 3 months after operation compared with pre-operation(z=-2.534,p<0.05;t=16.256,p<0.001;z=-5.724,p<0.001).Conclusion:1.The active-fluidics system of Centurion phacoemulsification instrument can maintain constant i ntraocular pressure during phacoemulsification,improve the stability of anterior cham ber,and make the operation safer and more effective.2.The effects of active-fluidics system and gravity-fluidics system on corneal endothelium in Centurion phacoemulsification are similar.3.The active-fluidics system can more effectively reduce the corneal edema and make the patients recover more quickly in the early postoperative period.
