| Background and Objective:Obesity is an important risk factor for hypertension,diabetes,cardiovascular disease and other chronic diseases.The study of obesity can provide a theoretical basis for the prevention and treatment of obesity,at the same time,it can also provide a corresponding theoretical basis for reducing related metabolic diseases or complications.According to metabolic indicators and obesity criteria,the population was divided into four states,namely,Metabolically healthy and normal weight(MHNW),Metabolically healthy obese(MHO),and abnormal body weight metabolism.Metabolically unhealthy but normal weight,MUHNW)and Metabolically abnormal obese(MAO),at present,there are not many studies on the MHO and MUHNW populations in China,and there are no reports in the Jiangsu population.The occurrence of obesity is the result of the combination of genetic factors and environmental factors,and fat-secreting adipokines play a vital role.As a kind of adipokines,adiponectin has different results in different studies from obese people with different metabolic types.Although the related research on adiponectin gene is extensive,there are few related studies on obese people with different metabolic types,and the conclusions have not been unified.To this end,relying on the Ministry of Science and Technology major research on diabetes,we want to conduct research on different metabolic types of obese people in Jiangsu,to understand the prevalence of different metabolic types of obesity in Jiangsu and related influencing factors,and to detect serum adiponectin levels and Genetic polymorphisms to explore its association with different metabolic types of obesity.Methods: The study design included cross-sectional studies and case-control studies based on cross-sectional studies.Epidemiological surveys and physical examinations were conducted in two districts of Jiangsu Province using multi-stage random sampling methods.To analyze the distribution of MUHNW and MHO in Jiangsu Province.At the same time,the population of 18 years old and above in the sixth national census in 2010 was used as the standard population to calculate the standardized prevalence rate.To compare the differences in physical examination indexes,clinical indicators and demographic characteristics between obese people with different metabolic types,and to analyze the influencing factors of obesity in different metabolic types.Serum adiponectin levels were measured by ELISA,and the differences in adiponectin levels between obese people with different metabolic types were analyzed.Case-control study was used to investigate the influencing factors of serum adiponectin levels and to analyze the relationship between adiponectin levels and obesity in different metabolic types.To analyze the polymorphisms of two adiponectin gene +45 and +276 in different metabolic types of obese people in Jiangsu Province,and to analyze the relationship between ADIPOQ gene polymorphism and obesity of different metabolic types.At the same time,the interaction between the two gene loci and environmental factors was analyzed in the addition model and the multiplication model.Statistical analysis was performed using SPSS22.0 software.According to the type of data and the needs of analysis,the chi-square test,Fisher exact probability method,T test,correlation analysis,multiple linear regression analysis and logistic regression were used.Results:(1)The standardized prevalence of metabolic normal obesity(MHO)and normal weight metabolic abnormality(MUHNW)in the 5167 survey population was 17.1% and 18.6% respectively.Age stratification revealed that the prevalence of MHO decreased with age,and the prevalence of MUHNW increased with age.Age,waist-tohip ratio and gender are related to the metabolically unhealthy but normal weight;in normal metabolic groups,age,waist-to-hip ratio and gender are the influencing factors of metabolic normal obesity;in obese people,age,waist-to-hip ratio,gender and alcohol are associated with metabolic abnormal obese.(2)The levels of serum adiponectin in obese people with different metabolic types were from normal control group(10.81±3.00μg/ml),MUHNW group(9.54±3.48μg/ml),MHO group(8.50±3.05μg/)and MAO group(7.01 ± 2.87 μg / ml).At the same time,the more aggregating abnormal components,the lower the adiponectin level.Multiple linear regression analysis suggested that BMI,SBP,WHR and TG may be the influencing factors of adiponectin.Logistic regression analysis indicated that low-level adiponectin was the influencing factor of MUHNW in normalweight population,and its OR value and 95% CI were 2.29(1.62,3.25).In the normal metabolic population,the risk of MHO in the group with low adiponectin levels was 3.19 times higher than that of high-level adiponectin,OR=3.19,95% CI=2.25~4.52.In obese people,those with low adiponectin levels were 2.58 times more likely to have MAO than those with high levels of adiponectin,and their OR values and 95% CI were 3.58(2.16,5.94).(3)Analyze the MUHNW population by comparing with the metabolically healthy and normal weight population.The results suggest that carriers of SNP+276 locus GG genotype are 2.03 times more likely to develop MUHNW than TT+GT genotype carriers,with an OR value of 95% CI of 2.03(1.43,2.86).In the interaction analysis,women with SNP+45 locus carrying the GT or TT genotype may have a significantly reduced risk of developing MUHNW compared with men carrying the GG genotype in the multiplicative model.In the analysis of the interaction between SNP+276 and drinking,compared with non-drinkers carrying TT or GT genotypes,GG genotype carriers and drinkers had a significantly increased risk of MUHNW in additive models and multiplicative models.Analyze the MHO population by comparing with the MHNW population.It suggests the carriers of SNP+45 locus GT and TT genotype were found to be 0.37-fold and 0.41-fold higher than individuals with GG genotype(OR=0.37,95% CI: 0.19-0.70;OR=0.41,95%).CI: 0.21~0.78)respectively.Combining the GT and TT genotypes,the risk of MHO in the population containing the GT or TT genotype was 0.42 times than that of the GG genotype population,OR=0.42,and 95% CI was 0.23~0.77.Analyze the MAO population by comparing with MHO population.The risk of MAO in SNP+276 locus GT genotype and GG genotype carriers was 3.24 and 3.13,respectively,and the OR and 95% CI were 3.24(1.54,6.79)and 3.13(1.51,6.50)respectively.Under the recessive genetic model,individuals with GT or GG genotype had a risk of MAO of 2.99 compared with individuals with TT genotype,OR = 2.99,95% CI: 1.49~5.99.In the interaction analysis,the SNP+45 locus interacts with gender in the multiplicative interaction.It suggests women with GT genotype or TT genotype have a greater risk of developing MAO.In the multiplicative interaction between SNP+276 and those with low serum adiponectin levels carrying GG genotype or GT genotype had a lower risk of MAO.Conclusion: 1.Men over 40 years old and those with excessive waist-to-hip ratio are associated with MUHNW and MAO.Young men with excessive waist-to-hip ratio are associated with MHO disease and should be paid attention to.2.There is a difference in the distribution of adiponectin between obese people of different metabolic types.Low levels of adiponectin are associated with obesity of different metabolic types.3.The G allele of the adiponectin gene SNP+276 is a susceptibility gene of MUHNW and MAO,and the SNP+45 site T allele is a susceptibility gene of MHO.The former interacts with drinking and,and the latter interacts with gender. |
