[Objective](1) To investigate the correlation between serum High molecular weight adiponectin (HMWAPN), leptin/HMW adiponectin (L/HA) ratio, leptin/adiponectin (L/A) ratio and metabolic syndrome (MS) in children and adolescents, and establish their roles in the diagnosis of the disease.(2) To investigate the association between plasma High molecular weight adiponectin (HMWAPN), leptin/HMW adiponectin (L/HA) ratio, leptin/adiponectin (L/A) ratio levels similar to that of the non-obese and the metabolically healthy obese (MHO) phenotype in children and adolescents.(3) To identify genes influencing variation in plasma total and HMWadiponectin levels.[Methods]A total of 3520 children and adolescents between 6 and 18 years of age were selected from the subjects that had been recruited for 2004 survey conducted by Beijing Children and Adolescent MS (BCAMS) study. Adusted criteria issued by International Diabetes Federation (IDF) in 2007 were used to establish a diagnosis of MS. Enzyme-linked immunosorbent assay (ELISA) was used to determine serum leptin and adiponectin levels. Multiple comparisons were made between groups using analysis of variance (ANOVA) and the chi-square test. Pearson correlation analysis was performed to determine the correlation between HMWAPN, L/HA, L/A values and various components of MS; areas under the receiver operating characteristic (ROC) curve were also examined to determine the diagnostic values of HMWAPN, L/HA, L/A in the diagnosis of MS in children. Using logistic regression models to find risk factors of MHO. Using multivariate linear regression models to find that SNP was significantly associated with the concentration of APN.[Results](1) As the L/H、 L/A value increased, body mass index (BMI), waist circumference (WC), blood glucose (BP), fasting blood glucose (FBG), low-density lipoprotein cholesterol (LDL-C), and triglyceride levels (TG) increased progressively, as did central obesity (CO%), the severity of insulin resistance (IR%) became more severe, and MS detection rate (MS%) increased. However, HMWAPN changed in the other way.(2) In all weight groups, the greater the number of risk factors for MS, the higher the serumL/HA, L/A value.(3) The area under the ROC curve plotted using L/A was greater than both the area under the ROC curve plotted using L/HA level and the area under the ROC curve plotted using HMWAPN level.(4) MHO phenotype can account for 54.71 of all obese individuals, and the MHO group have higher ANP and HMWAPN, but lower LEP, L/HA levels than MUO group P<0.05.(5) Using logistic regression models, we found that MHO was significantly associated with the concentration of APN and LEP.(6) 5 of 7 loci associated with total adiponectin levels and 6 of them associated with HMW adiponectin levels.(7) CDH13 rs4783244 loci exhibited significant association at P=10-37-10-34 with adiponectin levels.[Conclusions]HMWAPN L/HA and L/A are associated with obesity and MS, and can distinguish MHO.The level of genes study laid foundations for reguluating function of APN and the target of drugs.
