Analysis Of Drug Utilization Status In 8 Public General Hospitals In Jilin Province

Objective:Public general hospitals,as the main body of public hospitals,are of public welfare and are also the focus of solving the public's “expensive and difficult to see a doctor”.The overall use of medicines and the current status of the use of essential medicines are not only related to whether the people have “medicines available for treatment”.It also reflects the availability of "rich money for medical treatment".In recent years,the high prevalence and high mortality of cardiovascular diseases have been endangering people's health,and the cost of cardiovascular drugs has continued to increase,and the cost of patient medical expenses has also increased.Therefore,this study focused on eight public general hospitals surveyed in Jilin Province.The study conducts drug utilization research from the following three aspects,which are drug general,essential drugs and drugs for cardiovascular system diseases.In order to understand the current status of drug use in public general hospitals in Jilin Province,to further analyze the use of basic drugs and cardiovascular system drugs,to find possible problems in the actual use of drugs,to provide opinions and suggestions for improving drug use,and to promote reasonable use of medicines.Methods:The study used a multi-stage stratified sampling method to collect data from 8 public general hospitals in Jilin Province.This study investigates the use of chemical and biological products and chinese patent medicine in 2017,including the generic name,specifications,packaging unit,manufacturer,unit price,sales quantity and sales amount.The study uses Excel to establish a database template,establish coding tables for different attributes and data types of each institution,and then clean the collected data,establish a standardized database,and apply SPSS 24.0 for statistical analysis.Result:1.The total sales amount of drugs was 1186.62 million yuan,of which Western medicine accounted for 77.88%,and proprietary Chinese medicine accounted for 22.12%.The total sales amount of drugs in the secondary hospitals was 438.92 million yuan.The total sales volume of drugs in tertiary hospitals was 747.70 million yuan.The top three drug sales amounts are: drugs for the alimentary tract and metabolism(24.86%),drugs for the blood and blood forming organs(17.14%),and nervous system(12.60%).The top three sales amounts of chinese patent medicine were: internal medicine(81.90%),oncology medicine(8.00%),and orthopedic medicine(2.84%).The top three sales amounts of western medicine were: sodium chloride injection(4.99%),insulin aspart 30 injection(3.62%),and thymopentin for injection(2.16%).The top three sales amounts of Chinese patent medicine were: Danhong injection(10.99%),Shuxuening injection(9.45%),and Shuxuetong injection(4.29%).2.The number of varieties used in the catalogue is 319,of which 225 are western medicines and 94 are chinese patent medicine.The number of varieties in the secondary hospital use list is 296.The number of varieties in the tertiary hospital use catalogue is 213.The sales amount of essential drugs was 230.61 million yuan,accounting for 19.43% of the total drug sales.The top three sales amounts of essential drugs were: drugs for the blood and blood forming organs(42.34%),drugs for the alimentary tract and metabolism(26.94%),and cardiovascular system drugs(8.16%).The top three sales amounts of different types of chinese patent medicine in basic medicines were: internal medicine(70.95%),ophthalmic medicine(16.54%),and orthopedic medicine(7.86%).3.The top three sales amounts of different types of cardiovascular system drugs are: drugs for the cardiac therapy(26.39%),lipid modifying agents(21.44%),and calcium channel blockers(14.92%).The top three drug sales amounts were: atorvastatin calcium tablets(11.25%),cinepazide maleate injection(8.66%),and rosuvastatin calcium tablets(8.51%).The top three medication frequencies are: agents acting on the renin-angiotensin system,calcium channel blockers and lipid modifying agents.There are 48 drugs with the ratio greater than 1.The ratios in the top 6 are: candesartan,furosemide,isosorbide dinitrate,spironolactone,captopril,and nitrendipine.Conclusion:1.The sales of these drugs account for a large proportion.They are drugs for the cardiovascular system,drugs for the alimentary tract and metabolism,drugs for the blood and blood forming organs,drugs for the antiinfectives for systemic use,and drugs for the nervous system.The sales of Chinese medicines for internal medicine and tumor medicine account for a large proportion of Chinese medicines.The proportion of injectables among the top 20 medicines sold by western medicine and chinese patent medicine is high.2.The utilization rate of the medicines in the public general hospitals surveyed was slightly lower.Secondary hospitals need to use more varieties of essential medicines.3.The sales of these drugs account for a relatively large amount.They are drugs for the cardiac therapy,lipid modifying agents,calcium channel blockers,and agents acting on the renin-angiotensin system.
