Clinical Data And Treatment Study Of 420 Patients With Localized Vitiligo

Objective To analyze the clinical data of 420 patients with localized vitiligo,summarize the clinical characteristics of localized vitiligo,and explore the safe and effective treatment of localized vitiligo.Methods From January 2013 to August 2019,the clinical data of 420 patients with localized vitiligo diagnosed in the dermatology clinic of a hospital were collected.The clinical characteristics of localized vitiligo were summarized by statistical analysis of gender,age of onset,location of onset,season of onset,course of disease,inducement,family history,concomitant disease,disease stage and treatment effect.At the same time,120 patients from September 2017 to August 2019 were treated with 308 nm excimer light combined with tacrolimus ointment.The clinical follow-up survey was conducted for 3 and 6 months to analyze the efficacy and safety of 308 nm excimer light combined with tacrolimus ointment.Results(1)Among the 420 cases of localized vitiligo,201 were males and 219 were females,There was no significant difference in incidence rate between men and women.The average age of onset was 17.43±14.92 years,58.8% cases were in children,and the average duration of disease was 24.54±52.09 months.(2)67.9% of the patients occurred in the head,face and neck.(3)The onset season was mainly summer.(4)132 patients(31.4%)had inducing factors,among which the most inducing factors were sun exposure,51 cases in total.(5)33 patients(7.8%)had a clear family history,the average age of onset was 13.90±9.88 years old,387 patients without family history had an average age of onset of 17.74±15.28 years old,the difference was statistically significant(t =-2.037,P = 0.047).(6)40patients(9.5%)were accompanied by autoimmune diseases or other related diseases,among which 7 patients(1.7%)were accompanied by thyroid diseases.(7)The effective rate(97.6%)and the apparent efficiency(73.8%)of 308 nm excimer light combined with tacrolimus ointment of regular treatment for 6 months were significantly higher than those of 3 months(P = 0.00),respectively,the difference was statistically significant(P = 0.00).The effective rate and apparent efficiency of head,face and neck were higher than that of trunk,limbs and external genitalia(P = 0.00).After 3 months of treatment,the effective rate and apparent efficiency of children were higher than those of adults(P < 0.05);after 6 months of treatment,the effective rate and apparent efficiency of adults were higher than those of children(P > 0.05).There was no significant difference between male and female patients in treatment(P > 0.05).Conclusion(1)Localized vitiligo was more common in children,head and neck,There was no significant difference between male and female.The incidence was high in summer.A few patients had family history.(2)308nm excimer light combined with tacrolimus ointment was safe and effective in the treatment of localized vitiligo,which was worth popularizing.
