Objective:1.Compare the prevalence of moderate-severe obstructive sleep apnea hypopnea syndrome(OSAHS)between patients with CAD and patients without CAD.Discuss the correlation between moderate-severe OSAHS and CAD.2.Observe the characteristics of coronary artery lesions in patients with CAD complicated with moderate-severe OSAHS.3.Analyze the correlation between AHI and the severity of coronary artery lesions in patients with CAD,and discussed the relevant mechanism between the two diseases.Methods:A total of 183 patients who underwent coronary angiography(CAG)and polymorpnography(PSG)monitoring in the First Hospital of Shanxi Medical University were selected from July 2016 to July 2019.Collect the patient's basic information and laboratory reports,meanwhile record the PSG and CAG test result,caculate the Gensini score on the basis of CAG result.The patients were divided into CAD group and CAG normal group according to the CAG result,the CAD group was divided into the no or mild OSAHS group(AHI?15 times /h)(the simple CAD group)and the CAD with moderate-severe OSAHS groiup(AHI > 15 times /h)according to the sleep apnea hypopnea index(AHI).Data processing and statistical analysis using IBM SPSS20.0atatistical software.Results:1.There are 72 patients were diagnosed with CAD and the other 111 patients are assigned to the normal CAG group among the 183 patients.The numbel of patients diagnosed with moderate-severe OSAHS by PSG are 54(75.0%)and 38(34.2%)in the two groups.The prevalence rate of moderate-severe OSAHS in the CAD group was significantly higher than that in the normal CAG group,and the difference was statistically significant.In addition,the CAD group was older than the normal CAG group,and the rates of smokers,hypertensives and diabetics were higher than the normal CAG group,There were statistically significant different between the two groups(P<0.05).However,there was no statistically differences between the two groups in gender,BMI,drinking history,blood lipid,the ratio of moderate-severe hypoxemia,blood routine,d-dimer,liver and kidney function(P >0.05).2.The rate of patients with multi-vessel coronary disease and the Gensini score in the CAD group with moderate-severe OSAHS were higher than those in the simple CAD group,there were statistically significant differences(P<0.05).3.The AHI index was positively correlated with the degree of coronary artery disease(the Gensini score)in patients with CAD(r= 0.543,P<0.05).4.The results of multi-factor Logisitic regression analysis suggested that after adjusting other risk factors of CAD,moderate-severe OSAHS was an important predictors of CAD(OR=4.695,95%ci = 2.341-9.416,P < 0.05).Conclusion:1.Moderate-severe OSAHS is closely related to the occurrence and development of coronary artery disease,and the prevalence rate of moderate-severe OSAHS in patients with CAD is more than twice as much as the normal CAG group.2.The Coronary lesions in patients with CAD complicated with moderate-severe OSAHS is more serious than that in patients with simple CAD,and the rate of multi-vessel disease is higher.3.The severity of OSAHS was positively correlated with the Gensini score in patients with CAD,and the higher the AHI index was,the more severe the degree of coronary lesions was.4.Moderate-severe OSAHS is an important risk factor of CAD and it has certain clinical value in predicting CAD.
