Objective:To analyze the correlation between platelet distribution width(PDW)and ovarian cancer,and to explore its value in the clinical diagnosis of ovarian cancer.Methods:From July 2017 to December 2019,128 patients with ovarian benign tumor and ovarian malignant tumor were collected from The First Hospital of Shanxi Medical University.Among them,61 cases were diagnosed as ovarian cancer and 67 were diagnosed as benign tumor.60 healthy individuals from the Physical Examination Center of The First Hospital of Shanxi Medical University were selected as the control group.We analyzed the difference of PDW value in above three groups retrospectively.The relationship between PDW and FIGO stage,pathological type,differentiation degree,metastasis,lymph node metastasis was also analyzed.At the same time,the characteristic of PDW before and after operation was analyzed.Results:1.There was no significant difference in age,BMI and HGB between three groups(P>0.05).The PDW,MPV,PLT,AFP,CA125 and HE4 had significant differences among the three groups(P<0.001).2.The results of logistic univariate analysis showed that PDW,MPV,PLT,CA125,HE4,AFP were significantly correlated with ovarian cancer(P<0.05).Logistic multivariate analysis showed that PDW,CA125 and HE4 were significantly correlated with ovarian cancer.Higher CA125(OR=1.042),HE4(OR=1.054)and lower PDW(OR=0.509)were risk factors for ovarian cancer.3.The difference of PDW between ovarian malignant tumor and ovarian benign tumor was statistically significant(P<0.001).The difference of PDW between ovarian cancer and healthy control group was significant(P<0.001),but there was no significant difference between ovarian benign tumor and healthy control group(P>0.05).4.The PDW of patients with ovarian cancer advanced stage(?-?)was lower than that of patients with early stage(?-?),but the difference was not statistically significant(P> 0.05).Compared with the patients without lymph node metastasis,without other organ metastasis and low-grade tissue differentiation,the patients with lymph node metastasis,with other organ metastasis and high-grade tissue differentiation had lower PDW,but the difference was not statistically significant(P>0.05).There was no significant difference in PDW among the three pathological types(P>0.05).5.The postoperative PDW values of ovarian cancer patients were significantly higher than the preoperative PDW values,the difference was statistically significant(P<0.001).There was no significant difference between the preoperative and postoperative PDW values of benign ovarian tumor patients(P>0.05).6.The ROC curve showed that the best cut-off value of PDW in distinguishing benign and malignant ovarian tumor was 13.9fL(sensitivity was 83.6% and specificity was 71.6%),AUC was 0.842,which indicated that PDW had a certain accuracy in diagnosis of ovarian cancer.When PDW was combined with CA125 and HE4,AUC increased to 0.968,the accuracy of diagnosis was better.Conclusion:1.The PDW value of patients with ovarian cancer is lower than that of patients with ovarian benign tumor and healthy individuals.2.The patients with ovarian cancer advanced stage(?-?),lymph node metastasis,other organ metastasis and high histological differentiation had lower PDW value than those with early stage(?-?),no lymph node metastasis,no other organ metastasis and low histological differentiation,but the difference was not significant.3.The PDW value of ovarian malignant tumors increased after surgical treatment.4.The PDW has certain diagnostic value for ovarian cancer.When PDW is combined with CA125 and HE4,the diagnosis effect is better.
