Objective:1.This paper analyzes the status and spatial distribution of oral deciduous tooth retention among students aged 12-15 years in Shanxi Province and studies its epidemiological characteristics.2.To study the effect of individual signs,social economy,living habits and other factors on the retention of deciduous teeth,to provide reference for oral clinical and preventive work.3.To study the effects of air pollution,vegetation cover and other environmental factors on the retention of deciduous teeth,and to provide reference for government departments to formulate public health policies such as oral health care and prevention.Methods:Epidemiological survey of deciduous tooth retention: according to the five geographical areas of east,west,south,north and middle,the city and city sampling design is carried out,with urban and rural stratification,according to the PPS sampling method,each district and city to extract one county(township),one district(city),a total of 10 counties and districts,each county(district)to extract 3 secondary schools.The data were collected by clinical examination combined with questionnaire.Descriptive statistical indicators(frequency,percentage)describe the distribution of the survey subjects,illness,etc.,the retention rate of deciduous teeth was compared by chi-squaretest,the number of deciduous teeth retained by nonparametric rank sum test,spatial heterogeneity analysis by q-Statistic test Analysis of statistical methods,statistical analysis using SPSS 22.0 software,statistical significance level of 0.05.Analysis of influencing factors of deciduous tooth retention: single factor analysis was carried out by independent sample t test or chi-square test.taking into account the correlation between environmental factors(rainfall,PM2.5 concentration,topography,annual product temperature),in order to avoid the influence of multiple collinearity between environmental factors on regression results,this paper introduces principal component analysis(PCA)method and combines Logistic regression analysis method to study the effect of non-environmental factors on deciduous tooth retention under the premise of controlling the influence of environmental factors confounding.Subsequently,in order to minimize mixed effects of non-environmental factors.in this paper,the effects of environmental factors(rainfall,air PM2.5 concentration,vegetation cover)on deciduous tooth retention were studied by propensity score matching(PSM)-correction method combined with Logistic regression analysis.Results:Results of the epidemiological investigation on the retention of deciduous teeth: the total rate of retention of deciduous teeth in adolescents aged 12-15 years in Shanxi Province was 11.09%,and the number of retention of deciduous teeth per capita was 0.24.The prevalence rates were 19.9%,10.76%,7.48%,6.17%,and the per capita retention number of deciduous teeth was 0.53,0.23,0.12 and 0.09,respectively.With the increase of age,the retention rate of deciduous teeth decreased gradually,and the number of deciduous teeth per capita decreased gradually.Children aged 12 had the highest retention rate of deciduous teeth in this survey,with a statistically significant difference between men and women(P<0.05),while the age increased to After age 13,there was no significant difference between men and women(P>0.05).The retention rates of five cities in our province were from high to low: Luliang(Jinxi)> Taiyuan(Jinzhong)>Jincheng(Jinnan)> Datong(Jinbei)> Yangquan(Jindong).The retention rate of deciduous teeth and the number of deciduous teeth per capita were statistically significant(P<0.01).However,between urban and rural areas,the retention rate of deciduous teeth and the number of deciduous teeth per capita did not differ significantly(P>0.05).The tooth position distribution of deciduous tooth retention in absolute order from high to low is :55,65,75,85>53,63>54,64,74,84>73,83>71,81,52,62>51,61,72,82.The number of deciduous teeth retention was different between men and women(F=7.66,P=0.006).And in the remaining deciduous teeth,nearly 50% of the remaining deciduous teeth of all ages have caries without treatment,the caries rate of deciduous teeth is high and the treatment rate is very low,and the number of tooth without caries filled accounts for less than 1%.Analysis of factors affecting the retention of deciduous teeth showed that:non-environmental factors,BMI,have seen teeth,the mother's high education is a protective factor;and only child,dental caries in the mouth as a risk factor.Specifically,the greater the BMI,the less likely it is to have deciduous teeth stranded,with a OR value of 0.928(95%CI:0.909~0.947);the higher the number of dental caries in the mouth,the higher the probability of retention of the deciduous teeth OR the value was1.272(95%CI:1.224~1.321);one-child deciduous teeth retention VS non-only child OR=1.218 times(95%CI:1.039~1.427);people who have seen teeth VS those who have not seen teeth OR=0.751(95%CI:0.655~0.861);mothers with a high school degree or above are less likely to have deciduous teeth than mothers with a low education OR0.598(95%CI:0.383~0.935).Environmental factors,PM2.5 concentration is a risk factor for deciduous tooth retention,and vegetation cover is a protective factor for deciduous tooth retention.The higher the concentration of PM2.5,the greater the risk of retention of deciduous teeth,with a OR value of 1.293(95%CI:1.246~1.342);the higher the vegetation coverage,the lower the risk of retention of deciduous teeth,with a OR value of 0.664(95%CI:0.451~0.978).Conclusion:1.The retention rate of deciduous teeth in adolescents aged 12-15 years in Shanxi Province is not optimistic,and the caries rate of residual deciduous teeth is high,and the treatment rate is low,which will seriously affect the oral health status of adolescents.We should strengthen the popular science propaganda of the replacement of deciduous permanent teeth,recognize the harm of the deciduous teeth not falling off,and see a doctor in time.Reduce the incidence of retention of deciduous teeth and improve the level of cavity health in the modern population.2.The retention rate of deciduous teeth in Shanxi Province is obviously different between different regions,which is generally decreasing from west to east.3.BMI index is high,the number of dental caries in the mouth is small,the young people who have seen the teeth are not easy to retain the deciduous teeth,and the whole body condition is closely related to the oral disease,so we should pay attention to the whole body health and oral health of the young people in time.An annual routine stomatology examination is necessary.4.The mother's education affects the child's oral health,the parents should pay attention to the child's deciduous tooth fall off condition,if discovers the accident,should see a doctor in time.5.Environmental conditions have a certain impact on general health and dental conditions,air pollution will increase the risk of retention of deciduous teeth.Shanxi,which is in the period of major economic and social reform and transition,should attach great importance to the problem of "pollution health ",prevent and control pollution in time,increase vegetation cover,and reduce the damage of pollution to health.
