Objective:to explore the effect of computerized cognitive behavior therapy(CBT)on cognitive function in patients with anxious depressive disorder,it provides electrophysiological data support for enriching the study of cognitive neural mechanism of anxiety-depressive disorder and computerized cognitive behavior therapy.Methods1.Fifty patients met the major depressive disorder criteria(current episode)of Diagnostics and Statistical Manual of Mental Disorders 5~thh edition(DSM-5)and total scores of Hamilton Depression Scale(17 items)more than 17 scores and anxiety somatic factor more than 7 scores were randomly divided into research group and control Group.2.Repeatable battery for the assessment of neuropsyehological status were carried out on the participants, the total score of the scale and the score of each factor were analyzed, and P300,P50,N400 and N170 of the evented releated potentials were collected at the same time, the analysis index was P300,N400,N170 latency and amplitude and P50 ratio.3.The research group was treated with Mirtazapine combined with CCBT for 8 weeks, the control group was treated with Mirtazapine for 8 weeks, hamilton depression scale, repetitive neuropsychological state test and P300,P50,N400,N170 were collected at baseline and after treatment, respectively, the data were analyzed by independent sample t-test,chi-square test and paired sample t-test.Results:1.Analysis of clinical efficacyHAMD scores in both the study group and the control group were significantly lower than the baseline after treatment(P<0.05).After treatment, there was a significant difference in HAMD scores between the two groups(P<0.05).The score of the study group was lower than that of the control group.2.Cognitive function test analysisThe total score, immediate memory, verbal function, attention and delayed memory scores in the experimental group were significantly higher than the baseline after treatment(P<0.05),and the total score, immediate memory, attention and delayed memory scores in the control group were also significantly higher than the baseline after treatment(P<0.05).After 8 weeks of treatment, there were significant differences in the total score, visual span and attention factor between the two groups(P<0.05),and the score of the study group was higher than that of the control group.3.Event-related potential index analysis3.1 comparison of latency and amplitude of P300After treatment, the incubation period of P300 in the two groups was significantly shortened at the PZ,CZ and PZ electrode points, and the amplitude was also significantly increased(P<0.05).After 8 weeks of treatment, the incubation period of P300 in the study group was shorter at the PZ,CZ and PZ electrode points than that in the control group(P<0.05),with no difference in the amplitude(P>0.05).3.2 P50 ratio analysisAfter treatment, there was no significant difference in the P50 ratio at the CZ electrode point between the two groups(P>0.05),but there was a decrease in the ratio after treatment, and there was also no significant difference between the two groups(P>0.05).3.3 comparison of latency and amplitude of N400After treatment, the incubation period of N400 in the study group was significantly shortened at the electrode points of FZ,CZ and PZ(P<0.05),while the amplitude of N400increased at the electrode points of PZ(P<0.05).After treatment, the incubation period of N400 in the control group was significantly shortened at the electrode points of FZ,CZ and PZ(P<0.05),with no difference in amplitude(P>0.05).After 8 weeks of treatment, compared with the control group, the incubation period of N400 at the electrode points of FZ,CZ and PZ was significantly shortened(P<0.05),with no statistical difference in amplitude(P>0.05).3.4 comparison of latency and amplitude of N170After treatment, the incubation period of N170 at the electrode points of P7,P8,PO7and PO8 was shortened and the amplitude of N170 increased(P<0.05).After treatment, the latency of N170 in the control group was significantly shortened at the P8 and PO7electrode points(P<0.05),and the amplitude of N170 was increased at the P7 electrode points(P<0.05).After 8 weeks of treatment, the incubation period of N170 in the study group was significantly shorter than that in the control group at the electrode points of P7,P8,PO7 and PO8(P<0.05),with no difference in amplitude(P>0.05).ConclusionComputerised cognitive behavioural therapy can effectively improve anxiety depression cognitive dysfunction, embodied in the notice, the overall visual breadth and cognitive function, reflects the advanced cognitive processing of event related potential P300,P50,N400,N170 significantly shorten the incubation period, the execution,memory,evaluation,decision-making,speech,central inhibition and face processing such as cognitive function improved significantly.
