| Objective: To explore the effect of computerized cognitive behavioral therapy on the cognitive function of melancholic depressive disorder,and to provide a theoretical basis for the clinical treatment of melancholic depressive disorder.Methods: Through DSM-5 and MINI Chinese version,the patients with melancholic depressive disorder who meet the enrollment conditions were selected as enrolled subjects,a total of 60 cases.They were randomly divided into two groups for 8 weeks of treatment:one group was treated with medication and computerized cognitive behavioral therapy,which was the research group;the other group was treated with medication,which was the control group.Collect the general demographic information of the enrolled subjects,and HAMD-17,RBANS scale assessment and event-related potential P300,P50,N400 and N170 indicators before and after treatment for the subjects in the two groups.The total score of the HAMD-17 scale,the total score of the RBANS scale and the scores of various factors,the latency and amplitude of the event-related potentials P300,N400 and N170 indicators,and the S2/S1 ratio of P50 were obtained.Perform statistical analysis on the general demographic information,scale scores and event-related potential indicators obtained.The analysis before and after treatment within the group was performed by a paired sample t test,and the comparison between the study group and the control group was performed by an independent sample t test.Results: 1.Analysis of clinical treatment effect.After 8 weeks of treatment,the HAMD scores of the study group and the control group were significantly lower than before treatment(P<0.05).After treatment,the HAMD score of the study group was significantly lower than that of the control group(P<0.05).For melancholic depression,the effect of medication and computerized cognitive behavioral therapy was better than that of medication alone.2.Score analysis of neuropsychological cognitive function.After 8weeks of treatment,the total score,immediate memory,speech function,attention and delayed memory of the study group were significantly higher than those before treatment(P<0.05).The RBANS score of the control group was in total score,immediate memory,speech Function and attention were significantly higher than before treatment(P<0.05),and the total score and speech function of the study group were significantly higher than those of the control group(P<0.05).3.Analysis of event-related potential indicators.(1)P300: After 8 weeks of treatment,the latency of P300 at the FZ,CZ,and PZ electrode points of the study group and the control group was significantly reduced,and the amplitude increased(P<0.05);compared with the control group,the latency of the P300 of the study group was significant It was smaller than the control group(P<0.05),and the amplitude difference was not statistically significant(P>0.05).(2)P50: After 8 weeks of treatment,the P50 ratio of the study group and the control group decreased,but compared with before treatment,the difference was not statistically significant(P>0.05);compared with the control group,the difference was not statistically significant(P>0.05).(3)N400:After 8 weeks of treatment,the latency of N400 in the study group at the CZ and PZ electrode points was significantly reduced(P<0.05),and the latency of N400 in the control group at the FZ and CZ electrode points was significantly reduced(P<0.05),with a difference in amplitude No statistical significance(P>0.05);compared with the control group,the incubation period of N400 at CZ and PZ electrode points in the study group was significantly less than that of the control group(P<0.05),and the amplitude difference was not statistically significant(P>0.05).(4)N170: After 8 weeks of treatment,the latency of N170 in the study group at P7 and P8 electrode points was significantly reduced(P<0.05)and the amplitude was significantly increased(P<0.05),while the latency of N170 in the control group at P7 electrode points was significantly reduced(P<0.05),the amplitude at the P8 electrode point was significantly increased(P<0.05);compared with the control group,the latency of the N170 in the study group at the P7 and P8 electrode points was significantly reduced(P<0.05)but the amplitude was different No statistical significance(P>0.05).Conclusion: Computerized cognitive behavioral therapy has a good effect on the intervention of melancholic depressive disorder.It is helpful to improve the cognitive function level of melancholic depressive disorder,including memory,speech,attention and face recognition.It can be used in clinical treatment of melancholic depression.Promote the use of depression. |
