Objectives: To explore the reasons,prevention and therapy of deep brain stimulation(DBS)complications after surgery and analyze its clinical effectiveness.Methods :Retrospectively studied 42 patients with Parkinson’s disease who received DBS treatment in the department of neurosurgery of the First Affiliated Hospital of Xiamen University between June 2009 and August 2018 and analyzed its surgery complications,treatment and clinical follow-up effectiveness.Among them,25(59,5%)were male and 17(40.5%)were female,aged 40-74 years old,with H&Y rating of 2-4.Results :Complications related to surgical operation,target localization and stimulation occurred in 2 patients: 2 patients of cerebral hemorrhage(,4.7%)3 patients of electrode displacement,and 3 patients of incision skin ulceration.Among them,35 patients followed up to 18 months.35 patients were followed up for 18 months.Compared with preoperatively,the average H&Y grade of the 35 patients in the enrolled group was 3.2±0.7 when they were not taking drugs preoperatively,and 2.5±0.5 when they were taking drugs preoperatively.After 18 months of follow-up,the H&Y grade of the patients was 2.0±0.4 when they were taking drugs after starting up,and 2.4±0.5 when they were not taking drugs after starting up.After statistical analysis,it was found that there were statistically significant differences between the patients in the state of drug taking at the start of the machine,the state of drug taking at the start of the machine and the state of drug taking before the operation(P<0.01),and there were no statistically significant differences between the patients in the state of drug taking before the operation and the state of drug taking before the operation.Compared to pre-operation,Hoehn&Yahr(H&Y)grading,Unified Parkinson’s Disease Rating Scale(UPDRS)III total motor score,tremor,compulsion,bradykinesia and axial symptoms with or without medication before operation were significantly improved,which were statistically significant.Conclusions :1.Correct preoperative judgment,sufficient preoperative preparation,skilled operation of the operator,accurate electrode positioning,intraoperative neuroelectrophysiological detection,and the application of imaging fusion technology can reduce the occurrence of surgical complications.2.Bilateral STN-DBS electrode implantation can effectively improve the symptoms of muscular rigidity,bradykinesia,tremor,etc.,among which,the improvement rate of tetanus is the highest and the improvement rate of central axis is the lowest.3.Bilateral STN-DBS electrode implantation can reduce the dosage of anti-parkinson drugs in patients.
