Objective: To compare the clinical curative effect of the miniature steel plate,Herbert screw internal fixation in the treatment of Mason ? type radial head fractures.Methods: Following inclusion and exclusion criteria,collect Mason ? 35 cases of radial head fracture through Medical Records Room,making retrospective study.According to the internal fixation method,the patients were divided into two groups,group A and group B.Group A: were treated with miniature steel plate,contains 19 patients,aged between 18 and 65 years old,8 males and 11 females,6 left and 13 right,injury to operation is about 4 days.Group B: were treated with Herbert screws,contains 16 patients,aged between 18 and 65,11 males and 5 females,7 left and 9 right,injury to operation is about 4 days.Compare and analyze the hospitalization time,operation time,intraoperative blood loss,total time of drainage tube placement,postoperative drainage fluid,fracture healing time,postoperative complications,1 year postoperative Mayo elbow function score and elbow range of motion,different periods of Improved Broberg and Morrey elbow function score and visual simulation score.Results: All patients in group A and group B were made effectively follow-up visit for more than 12 months.There were no statistical significance in differences between the two groups in the time of hospitalization and total duration time of drainage tube placement(p>0.05).To operation time,intraoperative blood loss and total postoperative drainage fluid,group A>B,while in fracture healing time,group B <group A,the differences between the two groups have statistical significance(p<0.05).To the improved Broberg and Morrey elbow joint function score,in 3 months after the surgery,group A>group B,the difference between the two groups has statistical significance(p<0.05);in 12 months after the surgery,the difference between the two groups has no statistical significance(p>0.05).To the excellent and good rate of elbow joint,group A>group B at 3 and 12 months after surgery,but the differences between the two groups have no statistical significance(p > 0.05).To the visual simulation score,the differences between the two groups have no statistical significance at 1 day before surgery and 1 month after surgery(p>0.05),and the differences between the two groups has statistical significance at 1 day after surgery and 1 week after surgery(p<0.05).To the Mayo elbow function scores between the two groups in one year after surgery,the difference between the two groups has no statistical significance(p>0.05).To the elbow joint range motion in 1 year after surgery,the differences between the two groups have statistical significance in elbow joint pronation,pronation and flexion degree(p < 0.05).Complications: All patients in the two groups had no postoperative complications such as infection,nerve injury,poor fracture healing,elbow joint instability,and radius head ischemic necrosis.Conclusion: To the clinical curative effect in the treatment of Mason ? type radial head fractures,compared with Herbert screw,use miniature steel plate internal fixation,fracture healing time is relatively short,postoperative elbow motor function,elbow stability are faster recovered,elbow range of motion is better recovered,but,restricted by the “Security Zone”,relatively long operation time,relatively more intraoperative blood loss,relatively more amount of postoperative drainage fluid,postoperative pain relief is relatively slow.In conclusion,in choice of internal fixation of Mason ? type radial head fractures,should choose miniature steel plate as far as possible,if restricted by the “Security Zone”,should consider to choose Herbert screw internal fixation.
