Objective:To investigate the clinical value of nasal continuous positive airway pressure?n-CPAP?in children with severe bronchiolitis.Methods:With retrospective analysis,244 children with severe bronchiolitis were randomly divided into group A and group B.Group A was treated with nasal continuous positive airway pressure?n-CPAP?therapy on the basis of routine treatment?91 cases?,B group were given oxygen inhalation through nasal catheter or mask on the basis of routine treatment?153 cases?.The two groups were compared for their course of asthma,blood gas analysis,the time of relieving asthma,wheezing sound,average length of hospital stay and cost.Results:Group A of the time of relieving asthma and wheezing sound,and average length of hospital stay were significantly shorter than those in group A,but group A was significantly higher cost than that in group B,?they both P<0 05?.After treatment for 6 hours,12 hours,1 day and 2days,the respiration in group A was significantly better than that in group B?P<0.05?,after treatment for more than 3 days,the respiration was decreased in both groups,but there was no significant difference between the two groups?P>0.05?.PaO2,PaCO2 and SaO2 were significantly improved in group A and group B after treatment,and group A was better than group B?P<0 05?.In the 91 cases treated with n-CPAP,except for group e?Wheezing for more than 7 days before treatment?were significantly longer than those in other groups of the time of relieving asthma and wheezing sound,and average length of hospital stay?P<0 05?,there was no significant difference among the other groups?P>0.05?.Conclusion:The early application of n-CPAP in children with severe bronchiolitis can effectively alleviate the clinical symptoms,improve oxygenation and shorten the recovery time of symptoms.However,after giving oxygen therapy for more than 3 days,there was no statistical difference in the improvement of all indexes of the two respiratory patterns.Considering the economy and the matching degree of children,it is suggested to use nasal catheterization to provide oxygen.
