Background and purposeBronchitis (bronchiolitis), is a common inflammatory airway disease, more common in infants and children under 2 years of age, of which in 2-6 months baby incidence is highest. There were no significant differences between male and female incidence rate.The disease has obvious features of seasonal, multiple in spring and winter. The major clinical manifestations were cough, wheezing, inspiratory three depressions and auscultation of lungs wheeze, may have the severe complications such as espiratory failure, heart failure, water electrolyte disorders. As a kind of asthmatic disease, the pathogenesis of bronchiolitis and asthma are similar. Did RSV infection directly induce or cause asthma? Was bronchiolitis RSV infection just an early manifestation of asthma specifical fitness? At present it is debatable.Following up the children with bronchiolitis,30~39.17% of them would get asthma in the future. It affects the children’s physical and mental health and hinders children’s growth and development, and caused a certain degree of the family burden. In recent years, its incidence has a significant upward trend. The exact pathogenesis of bronchiolitis is not entirely clear. Most scholars believe that the virus infection is the important exogenous factor that occurs, In addition, environmental, genetic, immune disorderand nutrition and other factors may be the pathogenesis and bronchiolitis related diseases.During the development of capillary bronchitis, many kinds of immune cells, cytokines and chemokines play a role in the development of bronchiolitis. Chemokines are a class of small molecule polypeptide secreted, according to the 4 conserved cysteine residues of N terminal amino acid in the first two C with or without other amino acids, they can be divided into 4 subfamilies:the CXC class, CC class, CX3C class and C class.The corresponding receptors are CXCR, CCR, CX3CR and CR. Chemokines can chemotaxis of immune cells.They can also take part in the occurrence and development process in many physiological and pathological, such as inflammation, tumor, infection, autoimmune, allergic reaction, neovascularization and transplantation rejection.Chemokine receptor is mediated by G protein coupled chemokine exerts chemotactic function of 7 transmembrane receptor superfamily. Chemotactic factor plays a series of biological effects by binding to its receptors. The purpose of this study is exploring the possible pathogenesis of bronchiolitis by detecting the expression level of CXCR3 in the lymphocytes in peripheral blood and the serum concentration of its ligand Mig、IP-10、I-TAC about children with bronchiolitis.We hope that this research can provide reference for the clinical diagnosis and treatment of bronchiolitis.Materials and Methods1 The object of studyIn this experiment, we selected 55 cases of bronchiolitis as Group Maozhi.30 cases of bronchiolitis with allergic factors as bronchiolitis group Ⅰ.25 cases of bronchiolitis without allergic factors as bronchiolitis group Ⅱ.28 cases of normal children which having the same basic data were selected as Group Control. All children included in the experiment didnt have treatment history of immune drug one month before blood collection and they didn’t have history of other diseases resently. The control group of children didni have history of bronchiolitis.The children of Group Maozhi Ⅱ and Group Contral were no allergic constitution, and no personal or family history of asthma, allergic rhinitis and other history of allergic diseases. There were no significant differences in age, gender and living conditions between the 3 groups of the study.2 Specimen collection4ml venous blood of the three groups of children was collected before they got treatment.2 mL of fresh whole blood were centrifuged using EDTA-2Na. It is used to detect the expression of CXCR3 in peripheral blood lymphocytes of the surface, the other 2 ml of them did not have anticoagulant and centrifuged for 10 minutes at the speed of 3000r/min, then, we froze the serum under the condition of-80℃.3 The experimental methodThe expression of CXCR3 of the surface in the fresh peripheral blood lymphocytes was detected by flow cytometry. The levels of its ligands Mig, IP-10, Ⅰ-TAC were assayed with ELISA technique in their serum.4 The statistical methodThe results were demonstrated by means±standard deviation.we analyzed the results using one-way analysis of variance.Test among the three groups with statistics software SPSS 17.0.The mean differernce was significant at the 0.051evel and P value less than 0.05 indicated statistical significance.Results1 The results of CXCR3The percentage of CD4+T lymphocytes expressing CD 183 and the pencentage of CD8+T lymphocytes expressing CD 183 in both Group Maozhi Ⅰ and Group Maozhi Ⅱ were significantly higher than that in Group Control(P<0.05).The level of Group Maozhi Ⅰ was significantly higher than that of Group Maozhi Ⅱ(P<0.05).2 The results of MigThe Mig level in the Group Maozhi Ⅰ and the Group Maozhi Ⅱ was significantly higher than that in the Group Control(P<0.05).However no significant difference was found between the Group Maozhi Ⅰ and the Group Maozhi Ⅱ(P>0.05).3 The results of IP-10The IP-10 level in the Group Maozhi Ⅰ and the Group Maozhi Ⅱ was significantly higher than that in the Group Control(P<0.05).However no significant difference was found between the Group Maozhi Ⅰ and the Group Maozhi Ⅱ(P>0.05).4 The results of Ⅰ-TACThe Ⅰ-TAC level in the Group Maozhi Ⅰ and the Group Maozhi Ⅱ was significantly higher than that in the Group Control(P<0.05).And the level in Group Maozhi Ⅰ was significantly higher than that in the Group Maozhi Ⅱ(P<0.05).Conclusions1 CXCR3 and its ligands Mig, IP-10, I-TAC are involved in progess of bronchiolitis.2 In the pathogenesis of capillary bronchitis, allergic factors may affect expression of CXCR3, Ⅰ-TAC.
