OBJECTIVE: To observe the clinical effect of Percutaneous transforaminal endoscopy discectomy combined with Taohong Siwu Decoction in the treatment of lumbar disc herniation(qi stagnation and blood stasis syndrome),and observe the improvement of Taohong Siwu Decoction on postoperative low back pain symptoms and lumbar spine function evaluation Its clinical efficacy.Methods: In this study,60 patients with lumbar disc herniation(qi stagnation and blood stasis syndrome)were selected to be treated in the spine department of the Second Hospital of Fuzhou from January 2019 to September 2019 and planned to undergo percutaneous intervertebral foramenoscope.The patients were randomly grouped according to the order of visits according to the random number method and divided into control groups: percutaneous intervertebral foramen + conventional treatment.Experimental group: The control group was treated with oral Taohong Siwu Decoction plus or minus 3 weeks of treatment.Both groups of patients were given bed rest after operation and auxiliary functional exercise after operation.A total of 3 months of follow-up after operation.Through hospitalization,outpatient and telephone follow-up,the records were analyzed and compared with the VAS score,JOA score and ODI score of the patients before operation,1 day after operation,2 weeks,4 weeks and 3 months after operation.At the same time,3 months after surgery,the modified MacNab curative effect evaluation standard was used to evaluate the clinical treatment effect of patients.Results:(1)A total of 60 patients were enrolled and the follow-up period was 3 months.Among them,1 patient in the experimental group relapsed after 2 months and was switched to open surgery.Finally,59 patients were included in this study.Complete the research of this subject.There were 29 patients in the experimental group,with an average age of 40.10 ± 7.42 years and an average duration of 16.17 ± 2.35 months.The prominent segments were located at L3 / 4,L4 / 5,and L5 / S1 divided into 4 cases,15 cases,and 10 cases.There were 30 cases in the control group,with an average age of 38.60 ± 7.96 years and an average course of 15.33 ± 2.64 months.The prominent segments were located at L3 / 4,L4 / 5,and L5 / S1.There were 3 cases,18 cases,and 9 cases.(2)There was no statistically significant difference in preoperative VAS score,JOA score and ODI score between the two groups(P> 0.05),which was comparable.The difference between the VAS score,JOA score and ODI score of the two groups on the first day after surgery was statistically significant(P <0.05),and there was no significant difference between the scores on the first day after surgery between the two groups.(P> 0.05).The VAS score of the test group was significantly lower than that of the control group at each follow-up time point of 2 weeks,4 weeks and 3 months after operation.The difference was statistically significant(P<0.05).The JOA score was higher than that of the control group.The difference was statistically significant(P<0.05),and the ODI scores were significantly lower than the control group.The difference was statistically significant(P <0.05).The excellent and good rates of the treatment group and the control group at 3 months after surgery were 93.1% and 90.00%,respectively,and the difference was not statistically significant(P> 0.05).(3)None of the patients included in this topic experienced adverse reactions of oral Chinese medicine.Conclusion:Percutaneous transforaminal endoscopy discectomy combined with Taohong Siwu Decoction for the treatment of lumbar disc herniation(qi stagnation and blood stasis syndrome)plays an active role in early intervention.Taohong Siwu Decoction can improve the symptoms of postoperative low back pain in early patients.It is conducive to the functional recovery of patients after surgery and is worthy of clinical promotion. |
