Objective:To analyze the birth weight,the Proportion of gestational age,the distribution of common complications,hospitalization expenses and hospitalization time of premature infants in a grade 3 and first-class hospital in Enshi,so as to provide reference for clinical diagnosis and treatment and the establishment of reimbursement standard of medical insurance agricultural cooperative medical insurance.Methods:The questionnaire of complications of premature infants in NICU Affiliated Hospital of Hubei University of nationalities was designed to collect 774 live born premature infants who were born in January 2014 to December2018 and admitted to NICU within 24 hours as the research object.The data of gestational age,birth weight,complications,hospitalization expenses and hospitalization time were analyzed retrospectively.Results:(1)The sex ratio of prematurity was 1.2:1(422 male,352 female);according to the gestational age,16cases(2.07%)were<28 weeks,119 cases(15.37%)were from 28 weeks to 31+6weeks,182 cases(23.51%)were from 32 weeks to 33+6 weeks,457 cases(59.04%)were from 34 weeks to 36+6 weeks;according to the birth weight,17 cases(2.20%)were<1000g,99 cases(12.79%)were from 1000g to 1499g,and 1500 to 2499g 451cases(58.27%)and 207 cases(26.74%)were from≥2500g;(2)In this study,the top10 complications of premature infants in neonatal period were:381 cases of pathological jaundice(49.2%),392 cases of pneumonia(37.7%),269 cases of anemia(34.8%),240 cases of intracranial hemorrhage(31.0%),NRDS 211 cases(27.3%),156 cases of hypocalcemia(20.2%),108 cases of hypokalemia(14.0%),100 cases of neonatal asphyxia(12.9%),93 cases of feeding intolerance(12.0%),91cases of coagulation dysfunction(11.8)%;(3)The incidence of hyperbilirubinemia,pneumonia,anemia,NRDS,hypocalcemia,intracranial hemorrhage,neonatal asphyxia and feeding intolerance in different gestational age groups were statistically significant(P<0.05),while the incidence of hypokalemia and coagulation dysfunction was not statistically significant(P>0.05);(4)The incidence of pneumonia,anemia,NRDS,hypocalcemia,hypokalemia,neonatal asphyxia and feeding intolerance in different weight groups was statistically significant(P<0.05),while the incidence of hyperbilirubinemia,intracranial hemorrhage,coagulation dysfunction were not statistically significant(P>0.05);(5)The average length of stay was(15.53±13.43)days,the average cost of stay was(27522.13±25750.35)yuan;(6)There was a positive correlation between hospitalization cost and hospitalization time,R value was 0.781,and the difference was statistically significant(P<0.05);(7)Hospitalization days and hospitalization expenses were negatively correlated with gestational age,with R values of-0.543 and-0.572respectively,and the difference was statistically significant(P<0.05);(8)Hospitalization days and hospitalization expenses were negatively correlated with birth weight,with R values of-0.504 and-0.503 respectively,and the difference was statistically significant(P<0.05).Conclusion:The incidence rate of common complications of premature infants is closely related to gestational age and birth weight,the incidence rate of complications in different gestational age and birth weight is different,it suggests that the prevention and treatment of complications of premature infants should be carried out in different age groups and birth weight according to different gestational age and birth weight.The smaller the gestational age and the lower the birth weight will lead to the higher hospitalization expenses,which provides a certain reference for the establishment of medical insurance reimbursement standard for premature infants in Enshi.
