| OBJECTIVE: 1.To investigate the clinical complications of preterm infants with a gestational age of less than 32 weeks in department of Neonatology,Hunan Provincial People’s Hospital,in order to improve the treatment rate of premature infants..2.Collect clinical data of preterm infants with gestational age <32 weeks,explore the high-risk factors of bronchopulmonary dysplasia(BPD),establish a nomogram of the risk of bronchial dysplasia in preterm infants for early diagnosis and treatment of BPD,and improvement of prognosis.METHODS: 1.Collected the clinical data of preterm infants whose gestational age is less than 32 weeks from January 1,2017 to December 31,2020 in the Department of Neonatology,Hunan Provincial People’s Hospital,and conduct a retrospective analysis of the clinical data during the hospitalization of the infants,and record the preterm infants General conditions at admission,complications during pregnancy,related complications during hospitalization,and respiratory-related treatment measures.2.Divide premature infants into BPD group and non-BPD group,analyze high-risk factors of BPD,use 75% and 25% of the original data set as a modeling set and a validation set respectively,and use stepwise regression to screen independent variables to establish a simple predictive model(Stepwise Model).Finally,the receiver operator characteristic curve(ROC)is used to evaluate the early prediction performance of BPD.Results: 1.From January 1,2017 to December 31,2020,admitted to the Department of Neonatology,Hunan Provincial People’s Hospital,a total of 172 cases of preterm infants <32 weeks,including 95 males(55.23%)and 77 females(44.77%),gender ratio was 1.2:1,92 cases of normal delivery(53.48%),77 cases of cesarean section(44.77%),126 cases(73.26%)of single birth,46 cases(26.74%)of multiple births.Among them,the minimum gestational age is 26 weeks,and the minimum weight is600 g.2.The most likely cause of premature infants is premature rupture of membranes,66cases(38.37%);the complications of pregnancy during pregnancy are 34 cases of diabetes(19.77%)、24 cases of prenatal infection(13.95%)、and 21 cases of abnormal thyroid function(12.11%)、11 cases(6.4%)of hypertension during pregnancy.3.The common complications of children with gestational age less than 32 weeks were NRDS(57.56%),hyperbilirubinemia(53.80%),anemia(42.69%),and pneumonia(33.72%).4.RDS,asphyxia,apnea,pulmonary hemorrhage,BPD,premature infant encephalopathy,intracranial hemorrhage,etc.are related to gestational age(P<0.05).5.In this study,27 people developed BPD,with an incidence rate of 15.70%.Among them,premature infants with a gestational age of less than 28 weeks and a weight of less than 1000 g have the highest incidence of BPD.Gestational age,birth weight,Apgar score at 1 minute,number of fetuses,and use of prenatal hormones were related to the occurrence of BPD(P<0.05).Gender and delivery method have nothing to do with the occurrence of BPD(P<0.05).6.Gestational age,birth weight,1-minute Apgar score,multiple pregnancy,gastrointestinal bleeding,RDS,patent ductus arteriosus(PDA),and the use of invasive ventilator are independent risk factors for BPD.7.The combination of gestational age,RDS,and 1-minute Apgar score to construct a regression model can better predict the occurrence of BPD.Conclusion:1.Department of Neonatology,Hunan Provincial People’s Hospital from January 1,2017 to December 31,2020.The common complications of preterm infants with gestational age <32 weeks which in are RDS,hyperbilirubinemia,anemia,and pneumonia.2.Perinatal diseases of pregnant women with premature babies under 32 weeks include premature rupture of membranes,gestational diabetes,and prenatal infections.3.Gestational age,birth weight,1-minute Apgar score,gastrointestinal bleeding,RDS,PDA,and use of invasive ventilator are independent risk factors for BPD4.Gestational age,RDS,and 1-minute Apgar score can be used as the predictive indicators of the BPD prediction model,and the ROC curve reaches more than 0.80. |
