Objective Through three types of branch lesions in diabetic microvascular complications in Lanzhou City from 2013 to 2016: Diabetic Nephropathy(DN),Diabetic Retinopathy(DR)and Diabetic Peripheral Neuropathy(DPN)Patients' hospitalization expenses were used for information mining,and the hospitalization expenses of patients with three different complications were analyzed one by one;the new gray correlation method was used to analyze the internal structure correlation of the hospitalization expenses of patients with microvascular complications of these three types of diabetes.Simultaneously,starting from the sociological characteristics and clinical characteristics of patients,using the recursive system model to explore the various influencing factors of hospitalization expenses,quantifying the influence degree of each factor.Finally,the degree of influence between the influencing factors of hospitalized patients with diabetic microvascular complications is clearer.The study provides effective reference for the relevant health departments to control the rapid increase of hospitalization costs;prevent the incidence of diabetes microvascular complications increasing year by year,integrate health resources;improve the quality of medical services and utilization of health resources and decision-making.Methods The data comes from the GanSu province Health Commission's statistical information network direct reporting system.Through filtering and screening of diabetic microvascular patients from 2013-2016 in Lanzhou,a total of 12584 cases were obtained,including 5155 cases of diabetic nephropathy,1694 cases of diabetic retinopathy,and 5735 cases of diabetic neuropathy.In the basic descriptive analysis of patients' hospitalization costs,the continuous variables were selected for mean,median,standard deviation and quartiles.The categorical variables were expressed using the composition ratio(rate).Both the patient's hospitalization expenses and the hospitalization day did not follow the normal distribution,so the nonparametric rank sum test was used for analysis.This study uses 2016 as the base year to discount hospitalization expenses for other years using the Healthcare Consumer Price Index(CPI).The correlation degree between the individual expenses in the total hospitalization expenses and the total hospitalization expenses was analyzed by using the grey correlation degree.The SPSS22.0 was used to analyze the residual correlation of single factor analysis and finally the recursive system model was analyzed.The test level is ?=0.05.Results The number of patients with diabetic microvascular complications in Lanzhou increased from 2013 to 2016.The three types of complications showed that the number of male patients was significantly higher than that of female patients.The gray correlation showed that the hospitalization costs of the three types of patients were different.The top three nephropathy correlations were drug cost,comprehensive medical service fee,and diagnostic fee.The correlation degree was 1.0000,0.914022,and 0.880564;The top three correlations of diabetic retinopathy were drug,diagnosis,and comprehensive medical service.The correlation degree was 1.0000,0.919137 and 0.912826;the top three sizes of diabetic peripheral neuropathy were drug fee,comprehensive medical service fee,and diagnostic fee,the correlation degree was 1.0000,0.928260 and 0.912478.Univariate analysis and recursive system model indicated that the total effect of various factors on the hospitalization expenses of patients with diabetic nephropathy from the top to the bottom was hospital grade(level 3 hospital 0.531),hospital stay(0.363),whether surgery(-0.161),medical payment method(full self-fee 0.140)and the number of hospitalizations(0.089);the total effect of various factors on the hospitalization expenses of patients with diabetic retinopathy from the top to the bottom five in order of hospitalization days(0.438),medical payment methods(full self-fee 0.246)),hospital grade(three-level hospital 0.168),medical payment method(Xinonghe 0.153)and occupation(farmers 0.106);the total effect of various factors in diabetic peripheral neuropathy on hospitalization expenses from hospital to hospital Number of days(0.391),admission route(emergency-0.308),hospital grade(level 3 hospital 0.296),medical payment method(full self-fee-0.66)and occupation(technician 0.079).Conclusions 1.From 2013 to 2016,the average hospitalization cost of patients with diabetic nephropathy in Lanzhou was 6950.92 yuan,the average hospitalization cost for patients with diabetic retinopathy was 8653.10 yuan,and the average hospitalization cost for patients with diabetic peripheral neuropathy was 7805.87 yuan.The economic burden of patients was pretty heavy.2.To reduce the disease burden of patients with diabetic microvascular complications,it is essencial to focus on the population of males,farmers and technicians.3.Among the hospitalization expenses of patients with diabetic microvascular complications,the highest proportion of the three types of complications include drug charges,diagnostic fees and comprehensive medical service fees.Effectively reducing drug and diagnostic costs and increasing the value of medical staff is a focal point in effectively controlling hospitalization costs.4.The main influences of hospitalization expenses for patients with microvascular complications of three types of diabetes include the hospital level(third-level hospital)and medical payment method(all at their own expense).The number of hospital stays and surgery are also the main factors affecting the three types of microvascular complications.The recursive system model clearly manifests the complex hierarchical relationship between the various factors.It is proposed that the relevant health departments should start from the above main influencing factors and formulate more cost-effective measures to reduce the hospitalization expenses of patients with diabetic microvascular complications in Lanzhou.
