ObjectiveTo analysis basic epidemiological characteristics,composition of hospitalization expenses and trend of changes of patients with osteoporotic hip fracture in the elderly.we retrospectively collected data(including age,gender,fracture type,hospitalization time and cost,etc.)of elderly patients with hip fracture in 6 branches of two comprehensive hospital in Guangzhou(The Guangdong Province Traditional Chinese Medical Hospital,The Sun Yat-sen Memorial Hospital of the Sun Yat-sen University)from 1994 to 2014.MethodsClinical data of patients with osteoporotic hip fracture(femoral neck fracture or intertrochanteric fracture)were obtained from Medical record information management system of 6 branches of two comprehensive hospital in Guangzhou(The Guangdong Province Traditional Chinese Medical Hospital,The Sun Yat-sen Memorial Hospital of the Sun Yat-sen University)from in January 1994 to 2014 in December.Patient information include number,name,sex,age,date of admission,discharge date,duration of hospitalization,clinical diagnosis,surgery,hospital costs,bed costs,the cost of Western medicine,the cost of traditional Chinese medicine,check-in charges,treatment cost,surgical cost,laboratory cost,nursing cost,blood transfusion and oxygen cost,and other expenses.ResultsIn the past 21 years,with the development of social aging and urbanization in our country,the number of hospitalized patients with hip fracture has been greatly improved.In our data,the male to female ratio was about 1:1.92.The average age was 76.55±9.60(median 78)years old,male(75.47 ± 9.93,median 78)years old,female(77.12± 9.37,median 77)years old.The average age increased from 73.57±8.55(median 74)to 77.93± 10.29(median 80)years old over the past 21 years.The average LOS was about 22.59 ± 12.28(median 20)days with no significant difference between male and female patients,and with different diagnoses,but LOS decreased steadily from 30.55 ± 16.34(median 27)days in 1994 to 16.09 ± 7.34(median 15)days in 2014.The age group containing the most inpatient is 75~79 year old group(21.29%),subsequently is 80~84 year old group(20.18%),70~74 year old group(15.7%).Between 50~84 years old femoral neck fracture patients than femoral intertrochanteric fractures,and 85 years of age or older patients less than femoral intertrochanteric fracture.Between the ages group of 85~89 years,the two types of fractures had almost the same incidence.Average inpatient cost was about 26619.17 ± 20278.12(median 23231.46)Yuan(¥).The cost of hospitalization for patients with femoral neck fracture was(28579.74±21733.29,median 25603.54)Yuan,and the cost of hospitalization for patients with intertrochanteric fractures was(24257.83±18096.40,median 21711.52)Yuan.The cost increased every year by 10.16%over the past 21 years.The inpatient costs included 42.72%for treatment costs(that included implants,materials used in the operation and routine care of the wound),24.27%for medication,13.07%for operation.ConclusionThe results show that with the economic growth of our country,the cost of hospitalization for elderly patients with osteoporotic hip fracture is increasing.Among them,the cost of surgical treatment,intraoperative use of equipment costs(plants and materials)and drug costs play a leading role.According to the specific circumstances of patients,a reasonable choice of treatment plan will help reduce the cost of hospitalization for hip fractures.
