Objective:To analyze the relationship between the serum ferritin level and acute cerebral infarction,and to explore the relationship between serum ferritin and TOAST/OCSP classifications,and the relationship between serum ferritin and severity of acute cerebral infarction and short-term prognosis,respectively.This will provide some basis for early clinical prediction of cerebral infarction,severity evaluation,prognosis and treatment.This will provide a basis for clinical evaluation of the severity and prognosis of cerebral infarction.Methods:From February 2019 to December 2019,patients with acute cerebral infarction in the Department of Neurology of Shaanxi Provincial People's Hospital were prospectively and continuously included as the cerebral infarction group.At the same time,healthy physical examinations in outpatient or in-hospital Department of Shaanxi Provincial People's Hospital were taken as the control group.Patients in the cerebral infarction group were treated with subcomponent type.The national institute of health stroke scale(NIHSS)at admission was used to assess neurological deficits.Finally,the modified rankin scale(mRS)was used to determine the short-time prognosis during follow-up at 3 months.Two groups of general information and blood biochemical tests were collected.Serum ferritin was detected by radioimmunoassay.SPSS25.0 was used for statistical analysis.Results:1.Comparison of general data between cerebral infarction group and control group: there were no statistically significant differences in age,sex,history of diabetes,history of heart disease,smoking history,drinking history and BMI.However,the differences in the history of hypertension were statistically significant;2.Comparison of blood biochemical examination between cerebral infarction group and control group: there were no statistically significant differences in TC,TG and LDL-c.However,the differences among HB,WBC,UA and serum ferritin were statistically significant;3.Results of binary logistic regression analysis: UA and serum ferritin were independent risk factors of acute cerebral infarction;4.Comparison of serum ferritin in cerebral infarction group with different OCSP subtypes: there was no significant difference in serum ferritin between different OCSP subtypes(F=0.840,P=0.474);5.Comparison of serum ferritin in cerebral infarction group with different TOAST subtypes: there was significant difference in serum ferritin between different TOAST subtypes(F=3.265,P=0.024).Among them,serum ferritin in LAA group was higher than that in CE group and other groups and the difference was statistically significant.The differences TOAST subtypes were only statistically significant in UA(P<0.05);6.Comparison of serum ferritin in different NIHSS groups: serum ferritin in different NIHSS score groups was statistically significant(F=3.782,P=0.026).There was a position positive correlation between serum ferritin and NIHSS score(r=0.269,P =0.003);7.Comparison of serum ferritin in different mRS groups: there was no significant difference in serum ferritin between different mRS score groups(F=3.324,P=0.071);8.ROC curve analysis: the area under curve of UA was 63.8%,and when the UA level was 304?mol/L,the maximum youden index was 0.246.At this time,the sensitivity and specificity of UA in the diagnosis of acute cerebral infarction were 58.2% and 66.4% respectively,indicating that UA was a general diagnosis of acute cerebral infarction.The area under curve of serum ferritin was 78.5%,and when the serum ferritin level was 96.04 ng /mL,the maximum youden index was 0.426.At this time,the sensitivity and specificity of serum ferritin in the diagnosis of acute cerebral infarction were 76.2% and 66.4% respectively,indicating that serum ferritin was a good diagnosis of acute cerebral infarction.Conclusion:1.Serum ferritin and UA are significantly correlated with acute cerebral infarction and it can be regarded as independent risk factors of acute cerebral infarction;2.Serum ferritin and UA are related to LAA cerebral infarction,and there are some mechanisms involved in the occurrence and development of LAA cerebral infarction;3.Serum ferritin can be used as an indicator to judge the severity of acute cerebral infarction;4.Serum ferritin is valuable in the diagnosis of acute cerebral infarction.
