Objective To investigate the level of(Treg%Treg/CD4+T cell)and IL-6?IL-10?hsCRP in peripheral blood of patients with acute cerebral infarction of different TOAST subtypes,and to analyze the correlation between Treg% and inflammatory cytokines and TOAST subtypes.Methods 180 patients with acute cerebral infarction admitted to huainan first people's hospital from October,2017 to June,2018 were collected.According to TOAST classification,we divide cerebral infacrtion patients into 5 groups:72 patients with large-artery atherosclerosis(LAAS),38 patients with cardioembolic infarct(CEI),44 patients with lacunar infarct(LAC),22 patients with stroke of undetermined etiology(UDE)(excluded)and 4 patients with stroke of other determined cause(ODE)(excluded due to small number).45 heathly subjects were selected as control group.All the subjects had fasting blood drawn on the next day after admission.Tregs% was measured by flow cytometry and IL-6,IL-10 and hsCRP were measured by ELISA.Results 1.Treg% in control group was(7.40±1.63)%,and in acute cerebral infarction group was(7.39±1.67)%.There was no significant difference in Treg% between the two groups(p>0.05).2.Analysis of Treg% in each subgroup of cerebral infarction showed that Treg% in LAAS group was significantly lower than that in control group(p<0.05),and Treg% in CEI group was higher than that in control group(p<0.05).3.IL-6 level in cerebral infarction group was higher than control group.Among TOAST subtypes of acute cerebral infarction,Kruskal-wallis test showed that IL-6 level in CEI group was significantly different from that of LAAS group and LAC group(p=0.003,p<0.0001,respectively).4.IL-10 level in acute cerebral infarction group was lower than control group.Among TOAST subtypes of acute cerebral infarction,IL-10 level of CEI group was significantly from that of LAAS and LACgroup(p=0.001,p<0.0001).5.The level of hsCRP in acute cerebral infarction group was higher than that of control group.hsCRP level in CEI group was higher than that of LAC group(p=0.028).6.Logistic regression analysis show treg% in patients with acute cerebral infarction was negatively correlated with LAAS subtype(OR 0.125,95%CI0.039-0.405).Conclusion The level of Treg%,IL-6,IL-10 and hsCRP was significantly different among TOAST subtypes of cerebral infarction.In the TOAST subtype,Decreased Treg% can be used as a risk factor for LAAS,and it is an indicator of the etiology of early diagnosis of cerebral infarction.
