Objective: In order to further understand the prevalence of HCV infection in blood donors in nearly 30 years interval periods and to explore the factors that affect the outcome of its infection.Methods: In the rural areas of Jiangsu Province,some natural villages with paid blood donation in the late 1980 s and early 1990 s were selected.Using the cross-sectional field survey method,the researchers conducted a questionnaire survey and physical examination and detection of hepatitis C antibodies,simple blood routine and biochemical indicators in March-May 2016.Compare different blood collection methods and hepatitis C infection status,clinical outcomes and influencing factors.The demographic and clinical characteristics of the three groups of blood donation methods,HCV-positive and negative blood samples were compared by One-way analysis.Factors affecting anti-HCV positivity were analyzed by multivariate logistic regression analysis.This article judges the degree of liver fibrosis in patients by FIB-4.The demographic and clinical features of patients with mild hepatic fibrosis and severe liver fibrosis were compared by One-way analysis.Multivariate logistic regression analysis was used to analyze the risk factors for severe liver fibrosis in the FIB-4 index.Results: After a median 27 years(25~31 years)from last donated blood to survey,a total of 1694 participations were investigated,we observed that 12(3.3%),534(68.5%)and 324(58.8%)anti-HCV positive patients in blood donors,single plasma donors and mixed donors groups respectively.Multiple logistic(binary)regression analysis to assess that age,ALT,AST,PLT were associated with anti HCV positive.HCV RNA was detected in 702 cases(80.68%)of 870 anti-HCV positive patients,including < 104 IU/ml,88 cases(12.53%);> 104 IU/ml,< 106 IU/ml,432 cases(61.54%);> 106 IU/ml,182 cases(25.93%).The FIB-4 index of 702 HCV RNA positive chronic hepatitis C patients was calculated.The FIB-4 index was > 3.25,203 cases(28.92%),< 3.25,499 cases(71.08%).The demographic and clinical characteristics of the two groups were compared.The single-factor comparison showed that there were significant differences in age,drinking status,body mass index,PLT,economic status and number of donations between the two groups(all P < 0.05).There were no differences in sex,ALT,AST,education level and HCV RNA(all P > 0.05).The FIB-4 index was used as a binary variable to judge mild and severe hepatic fibrosis.Univariate P < 0.1 was used as a condition.The parameters such as age,drinking status,body mass index,PLT,number of donations and economic status were included in the logistic regression analysis.The results showed that age,especially over 60 years old,frequent drinking and PLT were risk factors for severe hepatic fibrosis judged by FIB-4 index.Conclusion: This interval investigation up to 30 years reflect chronic hepatitis C infection is very serious in the eastern China,single plasma donor and return to blood cells is the main way to cause hepatitis C infection.,Age,especially over 60 years old,frequent drinking and PLT are risk factors for severe liver fibrosis judged by FIB-4 index.
