Objective Through the screening of OBI in people who donated blood without compensation in rizhao city,the positive rate of OBI was calculated and the prevalence of OBI among unpaid blood donors in rizhao city was preliminarily analyzed.To explore the possible causes of OBI and the role of TGF-β1 in the occurrence of OBI infection,and to assess the residual risk of transfusions by blood transfusion,so as to provide ideas for the better formulation of policies,the improvement of blood screening strategies,and the provision of safe blood and blood products for clinical use.Methods(1)Routine blood test:117395 samples from free donors from 2014 to 2018were collected.Determination of HBsAg、HCV Ab、HIV Ag/Ab、TP in serum.To analyze the positive rate of HBsAg、HCV Ab、HIV Ag/Ab、TP in serum,and analyze the prevalence of transblood-borne virus in blood donors in the region.(2)NAT technology tested HBV DNA、HCV RNA and HIV RNA in blood samples:92110 samples tested by ELISA was selected,and NAT 8 people were mixed with the nucleic acid detection system of shanghai haoyuan and guangzhou daan.Statistics on HBV DNA、HCV RNA and HIV RNA positive rates were used to analyze the positive rates of OBI in people who donated blood without compensation.To study the prevalence of OBI in the region.(3)The“double half”test was carried out on the qualified HBV DNA specimens of HBsAg samples:31 cases of HBsAg negative HBV DNA positive specimens were detected in the laboratory since the start of the nucleic acid detection work,and hepatitis B“double half”was detected in this part of the sample,and the positive rate of anti-HBc antibodies in the sample was statistically analyzed.(4)PCR quantitative test:HBV DNA positive specimens were quantitatively tested to analyze their viral load levels.(5)The expression of TGF-β1 in serum of 30 healthy control group,30 cases of HBV group and30 cases of OBI group was detected by ELISA method.Results(1)In 117395 samples,2183 samples were found unqualified in laboratory tests,and the non-conformity rate was 1.86%.The overall non-conformity rate showed a declining trend year by year.There were significant statistical differences between years(c~2=709.00,P<0.05).Among them,249 cases of HBsAg unqualified samples had an unqualified rate of 0.21%,and the failure rate between years was statistically different(c~2=61.95,P<0.05).(2)A statistical analysis of gender,age and regions was conducted,with no statistical difference between the sexes(P>0.05);The failure rates of HBsAg for all age groups are 40.2%、23.3%、24.9%and 11.6%respectively,of which the failure rate between the ages of 18 and 25 is up to 40.2%.The comparative results between districts and counties were:38.2%、14.5%、26.1%and 21.3%,with the highest failure rate in Juxian was 38.2%,and there was no significant difference in other regions(P>0.05).(3)The positive rate of HBV DNA was 0.034%in 31 samples of 92110.HCV RNA did not detect reactive samples;HIV RNA was detected in 1 case with a positive rate of0.001%.(4)HBV DNA sample virus load is very low<20 IU/ml samples account for more than 70%.(5)The serum TGF-β1 level in the OBI group was increased compared with that in the healthy control group,with statistically significant difference(P<0.05).There was no statistically significant difference in serum TGF-β1 level between the OBI group and the HBV group(P>0.05).Conclusions(1)The OBI detection rate of unpaid blood donors in this region was 0.034%.(2)Nucleic acid detection applied in blood screening can shorten the“window period”of virus detection,which is of great significance for the detection of OBI,and can reduce the risk of hepatitis B virus transmission by blood transfusion.Nucleic acid detection and enzyme-free detection cannot replace each other.(3)New blood screening model electrochemical luminescence method plus nucleic acid detection is feasible for blood screening.(4)The immune factor TGF-β1 in serum plays an important role in the of continuous infection of OBI.
