Efficacy And Safety Of Shugan Jieyu Capsule With Paroxeting For Depression After Stenting

Objective With the widespread development of Peripheral Component Interconnect,PCI has brought revolutionary changes to the diagnosis and treatment of patients with coronary heart disease,especially acute myocardial infarction,which has greatly improved the survival rate of patients and the quality of life of patients.However,as a new stress event,percutaneous coronary surgery will increase the psychological burden of patients with coronary heart disease,leading to the occurrence of adverse emotions such as anxiety and depression.The purpose of this study is to explore the efficacy and safety of combined traditional Chinese and western medicine therapy(paroxetine combined with Shugan Jieyu Capsule)in the treatment of depression after stent,which will help further improve our existing treatment options.Methods Patients(Unlimited age and gender)who underwent PCI in our hospital from September 2017 to January 2020 and were treated in the Department of Cardiology of the People's Hospital of Inner Mongolia Autonomous Region.Coronary angiography showed single or multiple vascular disease,and the diameter of the diseased blood vessel ?2.0millimeter,stenosis> 80%,and rapamycin-eluting stents.Postoperative PHQ9 depression score were ensued.Eighty patients with mild to moderate depression were collected as study subjects.Collect general clinical data of patients(such as age,gender,history of hypertension,history of diabetes,history of hyperlipidemia,history of previous myocardial infarction,liver and kidney function,etc.).Then the patients were randomly divided into the experimental group and the control group.The combination of traditional Chinese and western medicine treatment group was given Shugan Jieyu Capsule combined with paroxetine.The control group was given paroxetine monotherapy.After 8 weeks,the patients were again assessed with the PHQ-9 depression scale.Check blood routine,biochemistry,liver and kidney function,electrocardiogram,and echocardiography.Then analyze the differences in treatment efficacy and adverse reactions between the experimental group and the control group,and evaluate the clinical application value of integrated traditional Chinese and western medicine for anxiety and depression after stenting.Results The effective number of paroxetine combined with Shugan Jieyu Capsule group(40cases)was 37,the effective rate was 92.5%,and the effective number of the control group(40cases)was 26,the effective rate was 65%.The difference was statistically significant(P<0.05);after treatment,the PHQ9 score of the combined Chinese and Western medicine treatment group was lower(2.875±2.35),and the control group(6.15±3.079).The difference was statistically significant(P <0.05).There was no statistically significant difference in cardiovascular adverse events between the two groups after treatment(P> 0.05).Among them,1 case of heart failure,3 cases of angina pectoris,and no myocardial infarction and sudden cardiac death occurred in the integrated Chinese and western medicine treatment group.In the contrast group,1 case had recurrent myocardial infarction,5 cases had angina pectoris,and 2cases had heart failure.No sudden cardiac death occurred.There were 2 cases of dry mouth symptoms in the integrated Chinese and western medicine treatment group,1 case of dry mouth symptoms and 2 cases of fatigue in the control group.There was no abnormal blood routine test results,no significant changes in cardiac ultrasound results,and no liver function or kidney adverse reactions occurred in the two groups.Conclusion Shugan Jieyu capsule combined with paroxetine can significantly improve the depression status of patients after stent surgery,thereby reducing the mental burden and quality of life,and avoiding additional economic pressure caused by repeated medical treatment;the curative effect of Shugan Jieyu capsule combined with paroxetine to Post-stent anxiety and depression are safe and reliable.Compared with paroxetine alone,the medicine combined therapy do not increase adverse effects such as liver and kidney damage.But,Compared with paroxetine group,the medicine combined therapy has not shown clinical benefits of significantly reducing cardiovascular adverse events.
