The Clinical Observation On TEAS With Rehabilitation Training In The Treatment Of Stroke Hemiplegia Lower Extremity Disorder

Objective Based on the data mining technology,to explore the acupoint selection rule of acupuncture in the current literature for the treatment of stroke hemiplegia lower extremity disorders,to observe the clinical effect of transcutaneous acupoint electrical stimulation on stroke hemiplegia lower extremity disorders,to provide data parameters and clinical evidence for the later research and development of transcutaneous acupoint electrical stimulation instrument,and to provide treatment ideas and methods for the clinical treatment of the disease.Methods 1.In literature research,CNKI,V2.0,VIP and Pubmed were searched by computer In Med,the clinical research literature about acupuncture and moxibustion in the treatment of stroke hemiplegia and lower extremity disorder was analyzed.The data were processed and the database was established.The frequency analysis,compatibility correlation analysis and complex network analysis of the relevant data were carried out by using Excel software,Modeler 18.0 and Gephi software respectively.2.In clinical research,60 stroke hemiplegia patients who meet the diagnostic criteria,inclusion criteria and exclusion criteria were randomly divided into the control group and the treatment group,30 cases in each group.The control group received 14 days of continuous rehabilitation training,and the treatment group received transcutaneous acupoint electrical stimulation on the basis of the control group.The results of statistical analysis of free hand muscle strength(MMT),Functional Independence Score(FIM)and lower extremity function score(LEFS)before and after treatment were obtained.Results 1.data mining results:(1)963 prescriptions of acupuncture and moxibustion were recorded in this study,including 267 acupoints,with a total frequency of 9948.(2)Frequency analysis results: the top 10 acupoints used was Zusanli,Hegu,Yanglingquan,Quchi,Sanyinjiao,Jianyu,Huantiao,Waiguan,Taichong and Shousanli;Five Shu Points,intersection points and original acupoints was used well;the main acupoints was hand Yangming large intestine channel,foot Shaoyang gallbladder channel,Foot Yangming stomach channel and foot Taiyin spleen channel;the acupoints of the upper and lower limbs and head and neck was the most in common use,acupoint selection has the characteristics of distance,up and down matching.The interventional time of electroacupuncture or electric stimulation was mainly 30 minutes,followed by 20 minutes;the commonest stimulus waveform was the density wave,followed by continuous wave;most of literature did not describe the stimulus frequency,and the recorded one was low-frequency stimulus,mainly concentrated below 20 Hz.(3)Results of acupoint association rules: Jie Xi and Zusanli are the most compatible,and Jie Xi,Zusanli,Xuehai,Sanyinjiao,Yanglingquan,Huantiao and Liangqiu have high-frequency compatibility;the connection between specific acupoints and Wushu acupoints is the most close;the connection between Sanjiao Jing,Danjing and dace Jing is close,followed by Sanjiao Jing,dace Jing,Danjing and Weijing Most of the points were taken from the upper and lower limbs.(4)The results of complex network analysis: the complex network of acupoint compatibility is a scale-free network model with the characteristics of small world network;after the degree analysis,Zusanli,Hegu,Yanglingquan and Quchi are the central acupoints of acupoint compatibility;K-core network analysis shows that there are 48 core acupoints,and the 10 core acupoints are Zusanli,Hegu,Yanglingquan Quchi,Waiguan,Sanyinjiao,Jianyu,Huantiao,Taichong,Neiguan.2.Results of clinical study:(1)60 patients were included in the study,56 patients were observed,and the loss rate was 6.67% within the scope of the study plan.(2)The statistical analysis showed that there was no significant difference between the two groups in gender,age and disease(P > 0.05).(3)There was the significant difference in MMT and LEFS before and after treatment(P < 0.05),and was statistical difference between groups after treatment(P < 0.05).(4)In the Functional Independence Score(FIM),there was the significant difference between the groups before and after treatment(P < 0.05),but there was no significant difference between the groups after treatment(P > 0.05).(5)In terms of safety: there was no difference in the basic signs and safety grading between the two groups before and after treatment(P > 0.05).Conclusion 1.The data mining results show that the points of Hand Foot Yangming,Shaoyang and foot Taiyin spleen channels is more frequently selected in the treatment of stroke hemiplegia.The core acupoints are Zusanli,Hegu,Yanglingquan,Quchi,Waiguan,Sanyinjiao,Jianyu,Huantiao,Taichong,Neiguan,Xuehai,Shousanli,Yinlingquan,Weizhong,Fengshi,Baihui,Kunlun,Xuanzhong,Fengchi and Liangqiu.Most of the acupoints were taken at the limb side of hemiplegia.2.The results of clinical study showed that the effect of transcutaneous acupoint electric stimulation combined with rehabilitation training on muscle strength and the motor function of hemiplegic patients with lower extremity disorders was significantly improved,and the lower extremity function of the affected side was improved,and the therapeutic effect was better than that of simple rehabilitation training.There were no adverse reactions in the two groups,which were easy to operate,reliable and safe.
